摘要:
An endobronchial blocker catheter (10) for use in blocking a selected mainstem bronchus (65) under guidance of a bronchoscope (64). The endobronchial blocker catheter comprises a tubular assembly and an elongated body that receives the tubular assembly. The tubular assembly comprises a tubular member (40) and a loop (45) disposed at a distal end of the tubular member. The loop is sized such that the bronchoscope is passable through the loop. The elongated body (12) has a plurality of lumens (28, 29, 30) therein, and an inflatable blocker balloon (17) disposed about a distal portion of the elongated body. One of the lumens (28) extends to an interior of the balloon to accomplish inflation of the balloon for retaining the blocker catheter in the mainstem bronchus, and another lumen (29) is sized for receiving the tubular assembly in a manner such that the loop extends from a distal end of the elongated body. The blocker catheter is guided by the bronchoscope to the desired inflation site via the loop.

A non-tracheal ventilation tube (10) includes a multi-lumen tube shaft (12) having a proximal end and a distal end (18), a proximal cuff (14) and a distal cuff (16). The multi-lumen tube shaft (12) is separated into a ventilation channel (20) extending from a proximal ventilation opening to a distal ventilation opening (32), a suction channel (22) extending from a proximal suction opening (42) to a distal suction opening (44), and an inflation channel (24). The proximal cuff (14) is attached to the tube shaft (12) and is connected to the inflation channel (24) for inflation and deflation. The distal cuff (16) is attached to the tube shaft (12) on the distal side of the proximal cuff (14) and is connected to the inflation channel (24) for inflation and deflation. The proximal suction opening (42) and the proximal ventilation opening are located on the proximal side of the proximal cuff (14). The distal suction opening (14) is located on the distal side of the distal cuff (16). The distal ventilation opening (32) is located between the proximal cuff (14) and the distal cuff (16). The outside diameter of the tube shaft (12) decreases between the proximal cuff (14) and the distal cuff (16) due to the ending of the ventilation channel (20).

An endotracheal tube (1) for selective bronchial occlusion for use in medical and surgical procedures in hospitals, and for on-site post-trauma medical stabilisation procedures, where it is often necessary to intubate the lungs with a view to isolating and occluding one lung and ventilating the other lung, the endotracheal tube (1) having a central channel (5) extending throughout its entire length with an opening at each of opposed distal (3) and proximal (4) ends thereof, the opening at the proximal end of the tube being adapted for connection to a ventilation device and the opening at the distal end comprising a first or distal air vent for venting air into the lungs, the bronchial/tracheal tube having separate spaced apart distal and proximal peripherally inflatable portions (6 and 7) at the distal end of the tube and at a proximal location relative thereto, respectively, and a second or proximal air vent opening (8) in the sidewall of that portion of the tube extending between the distal and proximal inflatable portions (6 and 7) wherein the distal inflatable portion (6) is inflatable both radially outwardly (as at 9) adapted to seal against the surrounding bronchus and radially inwardly (as at 10) to seal against itself to occlude the lumen of the tube (5) and wherein the proximal inflatable portion (7) is inflatable radially outwardly (as at 11) adapted to seal against the surrounding bronchial tube.

An inflation assisting implement has a check valve connectable with an injection tube, cuff inflating means which stores a gas or a liquid passing through the check valve and is operative to flow out the gas or the liquid therefrom, controlling means which controls the outflow of the gas or the liquid from the cuff inflating means to a specified flow rate, a stop valve which is normally closed and opened by a specified operation to flow out the gas or the liquid stored in the cuff inflating means, and a connecting part which feeds the gas or the liquid out of the cuff inflating means to a medical implement with cuff, in response to manipulation of the stop valve.

The object of the invention is to provide a stomach probe which makes a liquid-tight closure possible in the area of the oesophagus. The disclosed stomach probe is characterised by an inflatable stopper for closing the oesophagus in a liquid-tight manner. The inflatable stopper is made of a flexible and/or elastic material and forms at least one inner chamber closed to the outside for receiving a liquid medium. Means (11) are provided for adjusting a predetermined pressure for the medium in the inflatable stopper (16). An outer hollow tube (17) which extends into the inflatable stopper (16) is arranged on an inner hollow tube (17') which forms the actual probe, forming a channel between the outer tube (18) and the inner tube (17) in communication through a plurality of holes (20) with the inner chamber of the inflatable stopper (16) arranged on the outer tube (18). The inner chamber of the inflatable stopper (16) is in communication through the channel formed between the inner tube (17) and the outer tube (18) with means for generating the pressure in the inflatable stopper (16), i.e. with a preferably scaled reservoir or equalising vessel (11) for the liquid medium arranged above the inflatable stopper and outside the patient.

Lung volume reduction is performed in a minimally invasive manner by isolating a lung tissue segment, optionally reducing gas flow obstructions within the segment, and aspirating the segment to cause the segment to at least partially collapse. Further optionally, external pressure may be applied on the segment to assist in complete collapse. Reduction of gas flow obstructions may be achieved in a variety of ways, including over inflation of the lung, introduction of mucolytic or dilation agents, application of vibrational energy, induction of absorption atelectasis, or the like. Optionally, diagnostic procedures on the isolated lung segment may be performed, typically using the same isolations/access catheter.

An endotracheal tube (30) has an inflatable bag (40) towards its patient end for sealing with the inside of the trachea (11). Additionally, the tube (30) has a second, longer bag (50) separately inflatable to a lower pressure than the first bag (40) and such as to extend along the tube (30) from the first bag (40) to and through the vocal chords and larynx (13) and occupy the space between the first bag and the vocal chords (13). The inflated second bag (50) can thus reduce passage of secretions and minimises the space available for them. A suction line (26) extends along the tube (30), opening at a location (27) between, or at the interface between the two bags (40) and (50) so that any body secretions that do pass the second bag can be removed.

A passive continuous positive airway intratracheal pulmonary ventilation system which includes an inspiration limb (70) and an expiration limb (71) which are pressure balanced by inflatable bags (73a, 73b). An intratracheal pulmonary ventilation system is provided which includes a catheter (82) having a tip (90) which is positioned through a low resistance endotracheal tube (81) at or near a patient's carina (92). A constant source of an oxygen-containing gas flows through the catheter (82). The catheter has a tip (90) which deflects and reserves the flow direction of the oxygen-containing gas.
