    摘要: Apparatus of the present invention include encapsulating the antibiotic in fine particles of bioabsorbable polymer which will yield a constant dissolution of the antibiotic over a predefined dosing period (such as a 7-10 day period), with residual antibiotic being thereafter delivered only until the bioabsorable polymer is resorbed; and delivering the fine particle powder to the surgical site as an aerosol spray. It is preferable that the antibiotic and the polymer be thermostable so that the exothermic reaction of bone cement does not degrade the polymer or the antibiotic. A suitable antibiotic for this purpose is an aminoglycoside. One apparatus according to the invention includes a porous delivery tube which is dimensioned to fit comfortable in a bone canal and which is coupled to a aerosol canister containing the antibiotic powder of the invention. Another apparatus according to the invention includes a porous delivery tube which is preloaded with measured amounts of antibiotic powder and which is adapted to be coupled to a source of compressed air.

    摘要: A slotted patella resection guide (100) and stylus (400) used in the preparation of the human patella to accept a patella prosthesis in total knee arthroplasty. The guide includes jaw members (120, 121) for gripping bone, the jaw members including a plurality of calibrated and marked steps (slots 101-106) located at a different fixed predetermined height above the desired resection level. Each slot accepts the stylus which may be at least temporarily secured therein. After determining the amount of bone to resect, the stylus is temporarily secured in the appropriate slot (marked as being calibrated to yield the amount of resection desired); and the guide (110, 111) is then clamped onto the patella at the location indicated by the guide/stylus combination, with the stylus resting on the patella in the area where its thickness was measured to establish the depth of cut. After the guide is clamped in position, a sagittal saw is inserted therein to actually perform the resection. During resection, the stylus (400) may be removed from the guide to enhance visibility. The jaws of the guide include a saw capture slot (201a, 201b) which defines the plane of resection and accepts the saw blade to provide greater control of the cut during the operation. A saw blade insertion guide (150, 151) is also integrated into the resection guide to ease the process of inserting the blade into the capture slot and enhance safety characteristics of the instrument.

    摘要: A positioning tool (1) has an elongated cylindrical section (15) for reversibly engaging an acetabular cup (2), which is insertable through a posterior opening (28) in the cup. The outer diameter of a proximal portion of the cylindrical section (15) is expandable to engage the inner wall (30b) of the cup in an interference fit to prevent movement of the cup relative to the tool and hold the cup in engagement with the tool to move the cup with movement of tool to place the cup for implanting at a desired position. The expanded portion of the tool may be contracted to release it from the cup after implantation. The proximal portion of the tool comprising the cylindrical first section (15) and tapered section (19), which is attached to the distal portion and separated therefrom by a partial transverse slot (22). Third section (15) is divided into semicircular sections (34, 35) by a diametrical vertical slot (20) therebetween. Diametrically opposite quadrants (29a, 29b) of the semicircular sections (34, 35) further contain elongated slots (26, 27) therethrough which are perpendicular to the vertical slot (20) and spaced from the edge (57) of the cylinder by a thin section (25). The outer diameter of the proximal portion of the cylindrical third section (15) may be expanded or contracted by pivoting the semicircular sections (34, 35) around the thin spaces (25) between the elongated slots and edges of the proximal portions. The tool further comprises means for causing the semicircular sections to move relative to each other to expand or contract the diameter of the cylindrical section (15).

    摘要: A monomer dispenser and vial breaker (10) includes a base portion (12) and a cap portion (14) which is attachable to the base portion (12). The base portion (12) is substantially cylindrical, has an inner seat (16) for supporting the shoulder (2) of an ampoule (1), a knurled outer gripping surface (18), an exteriorally threaded portion (20), and an open end (22) with a first locking structure (24). The cap portion (14) is substantially cylindrical, has a first open (26) end with interior threads (28) and a second locking structure (30), a second open end (32), with a strainer and a vial breaking structure (34) located between the first (26) and second (32) open ends. The cap portion (14) is also provided with a knurled outer gripping surface (36). The monomer dispenser and vial breaker (10) operate as follows: an ampoule (1) is inserted into the base (12) so that the shoulder (2) of the ampoule (1) rests on the inner seat (16), the cap (14) is pushed onto the base (12) so that locking structures (24,30) engage, and the base (12) and/or the cap (14) are twisted relative to each other so that the threaded portions (20,28) engage. As the cap (14) and base (12) are threaded together, the vial breaking structure (56) breaks the bottom of the ampoule (1). The assembled device (10) is then inverted to dispense the monomer through the second open end (32) of the cap (14). After the dispenser (10) is assembled and the monomer is dispensed, the locking structures (24,30) prevent the device (10) from being disassembled.
