An improved apparatus and method of securing body tissue may be performed with a robotic mechanism. The robotic mechanism may be utilized to tension a suture with a predetermined force and urge a suture retainer toward body tissue with a predetermined force. Ultrasonic vibratory energy may be transmitted to the suture retainer to effect a gripping of the suture by the suture retainer. The body tissue may be secured with a staple. Legs of the staple may be bonded together to secure the staple. The legs of the staple may be bonded together by transmitting ultrasonic vibratory energy to the legs of the staple. A tissue positioning assembly may be used to hold the body tissue in a desired position. Images of the body tissue being secured may be obtained using various known devices including one or more endoscopes, a fluoroscope, a magnetic resonance imaging device, and/or other known imaging devices.

A passage through the anchor may be formed by flat inner side surfaces interconnected by inner corner portions. A suture is inserted through the passage. A concentrated force may be applied against a limited area on a trailing end of the anchor to rotate the anchor to move an outer corner portion of the anchor into engagement with body tissue. The suture may engage an inner corner portion of the anchor. The suture may be inserted through a plurality of anchors and the anchors moved through a tubular member into the body tissue under the influence of force transmitted from a trailing anchor to a leading anchor. An adjustable orthosis (10) for stretching tissue by moving a joint between first and second relatively pivotable body portions includes a first arm (24) with a cuff (28) at its outer end for releasably attaching the first arm to the first body portion, and a second arm (44) with a cuff (52) at its outer end for releasably attaching the second arm to the second body portion. The arms are pivotally connected at their inner ends. An actuator 22 is connected to the arms for applying force to the arms to pivot them relative to each other to move the joint. The actuator includes flexible force transmitting means (110) connected with at least one of the arms, and drive means (82) for applying force to the flexible force transmitting means to move the first and second arms relative to each other. In a preferred embodiment, the flexible force transmitting means is a rope or cable and the drive means is a manually actuatable winch for winding the rope or cable. The drive means is located at a distance from the pivotal connection of the first and second arms. The orthosis is adjustable while on the limb to a plurality of different positions including at least a first position in which the joint is flexed to a relatively greater degree and a second position in which the joint is extended to a relatively greater degree. An orthosis (10) for stretching tissue that limits compressive forces on the soil tissue around a joint during movement of the joint. The orthosis includes two relatively pivotable cuff arms (22,40) with a cuff (12,16) on each arm. The cuffs clamp onto the body portions on either side of the joint. The pivot axis (D) of the cuff arms is spaced from the pivot axis (A) of the joint, so that movement of the cuff arms to extend the joint results in distractive forces being applied to the joint. The cuffs are selectively moved on the cuff arms, during extension and flexion, to provide the proper amount of distractive forces to the joint and to limit compressive forces on the joint. A mechanical advantage is gained through the use of a gear drive mechanism for transmitting to the joint the force applied by the patient. This allows the orthosis to be relatively small and lightweight. The orthosis also provides a portable system for continuous passive motion therapy for a joint. The orthosis may be operated by a manual plus electric drive cycling it between extremes of motion. Provision is made for monitoring the range of motion or the force applied.

An orthosis for extension and flexion of a joint of a limb provides for distraction of the joint upon extension. In one embodiment, a spreading arrangement positively moves the orthosis arms longitudinally outwardly relative to each other upon rotation from a flexed position to an extended position. In another embodiment, the axis of rotation of the orthosis arms is located outwardly from the axis of rotation of the limb. Upon rotation of the arms relative to each other from a flexed position to an extended position, the orthosis arms move longitudinally outwardly because of the geometry of the orthosis. Because the cuffs on the arms are clamped to the limb segments, the limb segments are also forced outwardly, providing joint distraction. Structure is provided for limiting the amount of joint distraction and avoiding injury to the joint. The inner ends of the orthosis arms are linked together to maintain them in position relative to each other. The cuffs which attach the arms to the limb segments are adjustable longitudinally along the arms, to provide for different positions of the cuffs on the on limb, different length limbs, and the use of the orthosis on either a knee joint or an elbow joint.
