摘要:
Intervertebral disc replacement prosthesis consisting of a cylindrical cage and two securing pitch forks that can be placed in the inter-vertebral space to desired heights wherever in the Human Spine with anterior approach and, also on the Lumbar Spine with posterior approach in order to increase the total contact surface of the prosthesis. Only two instruments are demanded for its implantation and minimum time, it allows easy and constant optical checking of the whole procedure, due to less bleeding on the operating field and also due to the fact that no bulky instruments, guides etc. are used, and it finally combines the characteristics of both types of cages, cylindrical and flat ones, while it abolishes the disadvantages that each type has thus forcing surgeons to resort in positioning posterior fixation with pedicular screws at the same time for more safety.

Prosthesis for the arthrodesis of a human spine's segment of anterior and posterior (PLIF-ALIF) approach, consisting of two parts that are assembled during their implantation to the patient, interbody space after disc removal. For the anterior approach a conical threaded cage and conical forked cage, for the posterior appraoch a cylindrical threaded cage and two simple rectangular cages are used. Our prosthesis achieves the desired outcome when we tie two vertebrae, (arthrodesis), by distracting the space efficiently and stabilizing it with the screwing treaded cage, then enlarging the prosthesis surface by assembling the rectangular one. No need for further support with pedicle screws, as the previous techniques. Our prosthesis which combines both types has the characteristic that nullifies the disadvatages that existed on each type separately, that is migration, subsidence and abolishes the need for use of other support systems. Speed is also gained through simple positioning, avoiding errors and the dangers much than the previous techniques, while we expect a perfect outcome.

It is a telescopic cylinder(8,9)-plate (20) spinal prosthesis system for the replacement of one or more bodies of the spine, Lumbar, Thoracic or Cervical, including the disks, while it restores the height back to the normal in case of trauma injuries or other cause, and it secures and 'ties' in to four points with the upper and lower healthy body in to compression position, thus, preventing the substitute to be moved from its position due to shearing forces. This goal could be reached because it all acts, reacts and behaves as one solid unit. It also ensures cohesion and continuity of the graft which fills and bridges the gap between the healthy vertebral bodies so that the best conditions of its incorporation and secondary spine fusion will occur. Normally the high-grade stabilization it obtains may avoid the up until now need of a second posterior operation for the necessary additional stabilization with pedicular screws.

Posterior fusion system, screw-rod-connector, whose screw the body that is screwed in the bones is characterized by clockwise threads, helical attached to an anti-clockwise rotating central spring consisting of a core and anti-clockwise wings. The screw functions as an implant, since it is incorporated in the surrounding spongiosa bone completely, with the two rotated at 180 degrees helical compartments that allow communication and contact in a 360 degrees direction. Its front section is hollow and slightly pointed. Its connection is accomplished with a joint and divisible rod with free movement before the nut's final tightening, separately for each vertebra, thanks to two receptions of the joint and the rod's spherical heads. This eases the placement and also the rod's application without any tension and after tightening the nut it immobilizes it in any position and allows all reduction handling, before the final tightening. The whole system assembled in this way and with a flexible screw, which withstands more fatigue than the solid one, is incorporated much faster because the screw acts as an implant and thus withstands more fatigue. Finally, it increases the construction's longevity, reducing loosening danger from the implantation site.

Prosthesis for the replacement of a complete vertebral unit, whose assembly allows total removal and replacement of all the vertebral body's elements that are the pedicles with the joint facets and the lamina and has interconnection capability to a posterior fusion system which carries special screws 31 which has the capability of connection to a posterior fusion system, consists of the front part, which is of octagonal shape as in fig. 1 and special designed screws 31 that can connect it to the posterior fixation pedicle screw system and thus create a complete construction which aims at solving the problem of migration in cases of replacement of a complete vertebral unit. Hence, we avoid the danger of two independent fusions, which are currently used and are not stable enough when for some reason, usually tumor of the H.S., the laminas, the joints, and the pedicles have been destroyed besides the body. In this case, instability is great and demands minimization of migration's danger. The connection of the posterior pedicle fusion to the prosthesis at the front, and the new way of expanding its length with a toothed mechanism on the two ends where they touch the final plates, ensures quick insertion, and great stability which is necessary in operations of this type.
