摘要:
Implantable medical leads that are customizable and that are flexible and extensible in a controllable manner to facilitate subject body movements. In particular, implantable medical leads include the ability to be customized by selective and controllable separation of lead bodies from one another, which leads are also able to permit and withstand multiple degree of freedom of movement that are useful for use in the neck region of a subject body and other regions of any subject's body that may benefit from increased flexibility and extensibility.

Delivery of peripheral nerve field stimulation (PNFS) in combination with one o more other therapies is described. The other therapy delivered in combination with PNF may be, for example, a different type of neurostimulation, such as spinal cord stimulation (SCS), or a drug. PNFS and the other therapy may be delivered simultaneously, in an alternating fashion, according to a schedule, and/or selectively, e.g., in response to a request received from a patient or clinician. A combination therapy that includes PNFS may be able to more completely address complex or multifocal pain than would be possible through delivery of either PNFS or other therapies alone. Further, the combination of PNFS with one or more other therapies may reduce the likelihood that neural accommodation will impair the perceived effectiveness PNFS or the other therapies.

An implantable lead has a lead body construction designed to accommodate loading forces exerted on the lead body during patient movement. The lead body may be sufficiently stretchable to resist forces that could otherwise cause lead failure, axial migration of the electrodes, anchor damage, or tissue damage. Increasing stretchability of a lead body can also increase the vulnerability of the lead body to flex fatigue, buckling fatigue, kinking, and crush. Therefore, the lead described herein includes conductors that comprise coiled wires positioned substantially parallel to a center axis of the lead. The conductors described herein may be coiled around fibers that limit the axial stiffness of the coiled wires to ensure full recovery from axial loading.

There is disclosed a surgical lead which provides electrical stimulation to nerve tissue comprising two percutaneous lead bodies (12, 14) bonded together. The inventive lead body (10) results in the equivalent of a surgical lead body with columns of electrodes either adjacent to or offset from each other. A bridge (20) of urethane material is used to bond together the percutaneous lead bodies and still provide suitable flexibility of the lead. The surface of the electrodes may be coated, in part, with a suitable non-conductive coating (18) to effectively direct the electrical stimulation signals toward the targeted stimulation area. Significantly, the resulting lead is sized to fit within a needle having a similar cross-section, thereby permitting the percutaneous implantation of the inventive lead.

Increasing stretchability of a lead body can also increase the vulnerability of the lead body to flex fatigue, buckling fatigue, kinking, and crush. An apparatus and method for relative movement of electrodes in a medical lead are provided. The apparatus includes first and second electrode nodes and a wire connecting the electrode nodes. The wire includes a deformable sigma segment that allows movement of one or both of the first and second electrode nodes relative to each other. The method includes placing the medical lead adjacent tissue. The method further includes moving one of the electrode nodes relative to the other of the electrode nodes.

A medical electrical lead having an elongated lead body carrying multiple conductors (104, 106, 108, 110), wherein the lead body comprises: an inner core member (102) having radially extending grooves therebetween along the length of the core (102), an outer tubular member (100) surrounding the core, and conductors (104, 106, 108, 110) located in said one or more of the longitudinally extending grooves of the core. The dimension of the longitudinally extending grooves at the outer periphery of the core is less than the maximum internal dimension of the grooves.

A pulse stimulation system configured for implantation into a patient's body (28) comprises a pulse stimulator (102), a conductive stimulation lead (104) having a proximal end electrically coupled to the pulse simulator (102) and having a distal end, and an electrode assembly (122) coupled to the distal end of the stimulation lead. The electrode assembly comprises an electrode body having a therapy electrode (124) thereon that is electrically coupled to the stimulation lead for delivering therapy to the patient. A floating electrode (126) is configured to contact the patient's body tissue and has a surface area substantially larger than that of the therapy electrode. A filter (132) is coupled between the therapy electrode (124) and the floating electrode (126) for diverting RF energy toward the floating electrode (126) and away from the therapy electrode (124).
