Abstract: An intervertebral distraction tool has a clamshell head with upper and lower halves, each having a curvate outer surface and a flat inner surface. The distal side of the head is hinged so that the head opens and closes from the proximal side of the head. The hinge is a separating hinge that allows the halves to not only angulate with respect to one another about the hinge axis, but also to vertically separate from one another at the hinge. A distraction separator has decreasing taper at its distal end and a longitudinal bore that accommodates the elongate shaft of the trial so that the separator can be moved longitudinally relative to the shaft. Upon forward movement of the separator, the tapered upper and lower surfaces engage the flat inner surfaces of the head, causing the halves to angulate about the hinge axis of the head, thereby opening the head.

Abstract: A polyaxial orthopedic device for use with rod implant apparatus which includes a screw having a head, a tubular body having holes in the top, side and bottom therof, and a rod coupling element. The head of the screw is disposed in the body with the shaft of the screw extending out the bottom hole, such that the body and the screw may initially rotate relative to one another. The rod coupling element has a ball shaped end which seats in the body with the remainder of the rod coupling element extending out of the side hole of the body, such that the rod coupling element and the body are initially polyaxially coupled relative to one another. The ball end of the rod coupling element is disposed on top of the head of the screw. A set screw is provided in the top of the body, the tightening of which causes the ball, head, and body to be crush locked together, thereby preventing further relative motion.

Abstract: The present invention is a selectively expandable bone fixation screw-sleeve device which provides substantially greater holding strength in human bone than other bone screw devices. The sleeve element of this invention includes several embodiments, each having at least one corresponding screw element for selectively expanding a portion of the sleeve once it has been inserted into the bone. More specifically, in several embodiments, the sleeve includes a series of vertical slots in the end, or ends, of the cylindrical body of the sleeve. The slots define, therebetween, sections which are contiguous with the remainder of the cylindrical body, and which may rotate radially outward. In these embodiments, the sections are designed having a varying thickness, for example a single or double wedge profile, such that the insertion of the screw element into the sleeve deforms the shape of the sleeve by causing the sections to flair outward. In a different embodiment, the slots, and sections, are designed into the axially intermediate central portion of the cylindrical sleeve, and therefore flair outwardly therefrom upon insertion of a screw within the sleeve. As this invention is intended to provide imporved fixation support for orthopedic implantation assemblies, each embodiment may further include means coupled or integral with the sleeve or screw for coupling an orthopedic implant to it.

Abstract: A bidirectional rod locking mechanism for use with rod coupling elements of orthopaedic rod apparatus includes a rod coupling element having a hook-shaped rod receiving end and a threaded axial bore. The axial bore is disposed so close to the hook-shaped portion that the medial portion of the hole protrudes through the inner surface of the hook portion. A contoured threaded shaft is advanced into the bore such that the contoured medial portion thereof aligns with the inner surface of the hook to permit the receipt therein of a rod. The threadable translation of the shaft in the appropriate direction causes a misalignment of the contoured medial portion in a manner which prevents the rod from being removed from the hook-shaped receiving locus.

Abstract: A unitary locking cap mechanism, which is used with pedicle screws and other orthopaedic implant devices which couple bones to rods, includes rim and post portions which are rotationally freely coupled to one another to permit independent motion relative to one another. The rim element has a cylindrical body with an open end and a closed end; the closed end alternatively having a concentric hole defined by an inwardly directed flange. In one embodiment, the flange includes a series of slots, dividing into a plurality of wedge-shaped pie sections. In another embodiment, the sidewall of the cylindrical portion of the rim includes an annular recess. The post is a cylindrical shaft having surface elements at one end which provide for mutual engagement with the rim. These surface elements may be deflecting arms which seat into the annular recess of the rim, or an annular recess in the shaft into which the wedge-shaped pie sections of the flange portion may be resiliently snapped into. In an alternative embodiment, the shaft and the rim are coupled together through the concentric hole by a retaining ring which is placed around the shaft, in an annular recess therein, after the insertion of the shaft through the hole. Each embodiment are alternatively designed such that the post or the inner surface of the rim portions are threaded so that it can mate with a threading on the inner or outer upwardly extending, U-shaped channel forming, members of the heads of the pedicle screws.

Abstract: Craniotomy closures comprising surgical fasteners are described for use in reattaching a skull flap removed from the skull of a patient during brain surgery. Methods of using the same are also described. Surgical strips used in combination with the fasteners are shaped to follow the perimeter contour of the skull flap. Abstract: A driver assembly for affixing a surgical fastener to a target location is provided. Operation of the driver assembly inserts the surgical fastener in two stages, first an alignment stage through application of a distally directed force to partially insert the surgical fastener, and then a fastening stage to fully insert and seat the surgical fastener to a proper depth or compression level. The driver assembly comprises a spring loaded automatic trigger mechanism that may be adapted for use with a linearly insertable or a rotationally insertable surgical fastener. Application of the distally directed force actuates the trigger mechanism, wherein a corresponding impact force is delivered for seating the surgical fastener, coupled to a distal end of the driver assembly, upon alignment of cam and receiver elements embodied within the trigger mechanism.

Abstract: A biocompatible surgical fastener, comprising a sleeve and a screw or a pin, which expands when it is implanted in a patient. The sleeve is first implanted in a pre-drilled hole in the operating area of a patient, usually in a bone, cartilage or a bone and cartilage. When a screw or a pin is installed in the sleeve, the sleeve is caused to expand thereby stabilizing the fastener in the operating area. In many embodiments the screw or pin can be removed and reinserted without disturbing the tissue in the operating area. Abstract: Strip fasteners and cranial plugs for use in reattaching a skull flap removed during brain surgery and methods of using the same. The strip fasteners are flexible and can be shaped to follow the perimeter contour of the skull flap. The cranial plugs can be used to reattach the skull flap or they can be installed after the skull flap is reattached using the strip fasteners.
