    摘要: An implantable cardiac rhythm management device capable of automatically detecting intrinsic and evoked response of a patient's heart. The device operates in an automatic capture verification mode, wherein a global accelerometer signal is utilized to identify heart sounds (S1 and S2) of the patient's heart. The presence or absence of one or more of the heart sounds S1 and S2 in the accelerometer signal indicates whether a stimulation pulse evokes a response by the patient's heart. The device may automatically adjust the stimulation output in accordance with a step down stimulation protocol, wherein the presence of a predetermined heart sound indicates capture. Also, the device may suspend the automatic capture verification sequence if the patient's physical activity level exceeds a predetermined threshold.

    摘要: A cardiac rhythm management device having a capture verification sensing circuit for use in either a normal beat to beat pacing mode or an autothreshold mode. The cardiac rhythm management device is coupled to one or more pacing leads having pacing/sensing electrodes coupled thereto, and includes a sensing circuit for sensing electrical activity of the patient's heart, wherein the sensing circuit includes a dedicated evoked response sense amplifier and at least one high pass coupling capacitor electrically connected as a portion of a high pass network and between a pacing/sensing electrode and a blanking switch. A plurality of high pass coupling capacitors are coupled in parallel each having a separate blanking switch, thereby creating a differential network to offset imbalance sensed from the electrode due to extraneous factors.

    摘要: A system and method for cardiac rhythm management, which includes an electrode system having at least one electrode and control circuitry coupled to the electrode system from which a first cardiac signal is sensed. The control circuitry includes a pulse circuit to produce electrical pulses at a first value to be delivered to the electrode system in a first cardiac region. At least one cardiac signal is sensed from a second cardiac region, where the cardiac signal includes indications of cardiac depolarizations from the second cardiac region which occurs in direct reaction to the electrical pulses delivered to the first cardiac region. The first value of the electrical pulses are then modified by a pulse adjustment circuit when a cardiac depolarization which occurs in direct reaction to the electrical pulse delivered to the first cardiac region is detected from the second cardiac region.

    摘要: Methods and devices are disclosed for employing mechanical measurements to synchronize contractions of ventricular wall locations. Accelerometers that may be placed within electrode leads are positioned at ventricular wall locations, such as the left ventricle free wall, right ventricle free wall, and the anterior wall/septum wall. The accelerometers produce signals in response to the motion of the ventricular wall locations. A processor may then compare the signals to determine a difference in the synchronization of the ventricular wall location contractions. The difference in synchronization can be determined in various ways such as computing a phase difference and/or amplitude difference between the accelerometer signals. One or more stimulation pulses may be provided per cardiac cycle to resynchronize the contractions as measured by the accelerometers to thereby constantly and automatically optimize the cardiac resynchronization therapy.

    摘要: Various aspects provide an implantable device. In various embodiments, the device comprises at least one port, where each port is adapted to connect a lead with an electrode to the device. The device further includes a stimulation platform, including a sensing circuit connected to the at least one port to sense an intrinsic cardiac signal and a stimulation circuit connected to the at least one port via a stimulation channel to deliver a stimulation signal through the stimulation channel to the electrode. One such device is directed to a catheter, and uses thereof, for capturing myocardium of a heart by delivering pacing signals to a location in the heart. The location is near a His Bundle of the heart. The catheter has a proximal end for interfacing with an electrical pacing signal source and a distal end. The distal end includes a fixation mechanism that attaches the catheter to heart tissue. First and second electrodes are also located at the distal end. Each electrode is individually addressable for providing pacing signals to the heart tissue and also arranged to physically contact the heart tissue when the fixation mechanism is attached to the heart tissue.
