The present invention pertains to a magazine that is loaded with a needle and an attached length of suture pre-tied in a knot on the magazine. The magazine is used in loading the needle onto a stitching surgical instrument and in loading the tied length of suture onto a suture tying surgical instrument.

In the connection and encapsulation of substrates such as cables by covering members such as heat-recoverable sleeves, typically provided with adhesive or mastic linings, the formation upon heating of a desired hermetic seal between the substrate and the covering member is checked by using a device which can detect the excess air pressure which, once the hermetic seal has been made, builds up within a space between the covering member and the substrate. Preferably this is done by means of a probe which passes between the covering member and the substrate into said space and which is connected to a pressure sensitive device such as a balloon or a manometer. After use, the probe, which is preferably made from a plastics material such as nylon, can be fused or deformed to ensure that the hermetic seal remains intact.

Apparatus and methods for applying fasteners during endoscopic surgery are provided. The apparatus generally comprises a handle portion, a triggering mechanism and a fastener applicator. One embodiment of the invention forms the fastener around a single focal point on an anvil. Another embodiment vertically stacks the fasteners in a fastener applicator which is readily detachable from the handle portion by virtue of a rotational locking system and which may be replaced with a new fastener applicator having a fresh load of vertically stacked fasteners. The various embodiments may be variously combined together in a single device. The methods generally include the following steps: forming a fastener by placing it over a single focal point anvil and pressing the fastener against the single focal point anvil using a slide.

The suture retainer according to one aspect of the invention has a retainer body and a plurality of suture attachment mechanisms spaced circumferentially about the retainer body, each of the suture attachment mechanisms (such as a pair of teeth) being adapted to hold a suture strand at a discrete attachment point. Another aspect of the invention provides a method of securing a suture in a suture retainer, including the following steps: providing a suture retainer having a retainer body and a plurality of suture attachment mechanisms spaced circumferentially about the retainer body; and attaching a suture strand to the retainer body at each of the suture attachment mechanisms. Yet another aspect of the invention provides a suturing method including the steps of: attaching a first end of a suture strand to a suture retainer; contacting a tissue contact portion of the suture strand with tissue to be sutured, the tissue contact portion being disposed between the first end of the suture strand and a second end of the suture strand; and attaching the second end of the suture strand to the suture retainer at a plurality of suture attachment points spaced circumferentially about the suture retainer.

A method and apparatus for dissecting a first layer of tissue from a second layer of tissue and thereafter holding open an anatomic space for the performance of a surgical procedure. The method includes steps of making an incision in a body, introducing a deflated balloon dissector into the incision, inflating the balloon dissector to effect dissection of the first layer of tissue from the second layer of tissue, deploying a retractor within the anatomic space in order to hold open the anatomic space, and optionally deflating or evacuating the balloon dissector to open a cavity for surgical manipulations. The apparatus includes a combined dissector-retractor having a balloon retractor disposed upon the surface of the balloon dissector and integrated therewith.

Apparatus and method for applying fasteners during endoscopic surgery. The apparatus vertically stacks fasteners in a fastener applicator which is detachable and replaceable from a handle portion by a rotational locking system. The fasteners are applied by moving them to a driving channel by a biased fastener positioning spring as a slide is retracted; the slide is then advanced until it engages a fastener in the driving channel and drives the fastener over an anvil. During advancement of the slide a biased stop spring is forced back into a recess thereby allowing the next fastener to move forward in the storage channel; the slide is then retracted, freeing biased ejector springs to kick the formed fastener off the anvil; finally the slide is further retracted, until the fastener positioning spring is once again free to move the distal-most fastener from the storage channel into the driving channel.

An apparatus for applying a blood vessel clip includes a grip assembly having a handle, an applicator barrel attached to the grip assembly, a magazine connected to the applicator barrel, and a plunger connected to the grip assembly. The magazine has a plunger space and a magazine passage separated by a divider shim. A distal end of the magazine defines an exit aperture. The magazine passage is adapted for storing at least one blood vessel clip therein. The magazine includes a constricted region narrower than a head of the blood vessel clip. The constricted region is spaced apart from the exit aperture and has a narrowest point narrower than the exit aperture. The plunger extends into the plunger space. The plunger moves the at least one blood vessel clip distally through the constricted region and the exit aperture.

A balloon brachytherapy applicator has either a single or double chambers, with the capability of “blistering” outwardly at a specified area on one side of the balloon. This is useful to move sensitive tissue, such as skin or bone, farther away from a radiation source within a source guide inside the balloon. In one case a secondary balloon or secondary compartment is formed on the primary balloon, and is separately inflatable so as to allow blistering as desired when needed. In another form the balloon comprises a single chamber, primarily of inelastic or fairly rigidly elastic wall material, but with a “window” or area in which the balloon wall is elastic, or substantially more elastic than the main balloon wall, so that inflation of the balloon up to a certain point forms a substantially regular balloon shape (which may be approximately spherical), and the addition of further pressure will blister out the elastic area to increase the distance between the shaft and the sensitive tissue in a desired orientation of the balloon.
