Abstract: In an interventional medical system, a delivery catheter for deploying a medical device of the system has a tethering assembly that includes a tether line, a grip member, and a release member, wherein the tether line extends through a longitudinal lumen of a base of the release member and is coupled to a base of the grip member, and the release member has legs extending though apertures formed through the base of the grip member. The device may be tethered to the catheter such that a proximal end of the device is held within a cavity defined by a plurality of elastic fingers of the grip member that extend distally from the base thereof; and the device may be deployed from the catheter by moving the legs of the release member within the grip member cavity to push the proximal end of the device out from the cavity.

Abstract: A tine portion for an implantable medical device includes a hook segment and a distal segment terminated by a tissue-piercing tip, wherein the distal segment extends from the hook segment to the tip. The hook segment, which is elastically deformable from a pre-set curvature, has one of: a round cross-section and an elliptical cross-section, while the distal segment has a flattened, or approximately rectangular cross-section. One or a pair of the tine portions may be integrally formed, with a base portion, from a superelastic wire, wherein the base portion is configured to fixedly attach to the device, for example, being captured between insulative members of a fixation subassembly.

Abstract: VfA cardiac therapy uses an implantable medical device or system. The implantable medical device includes a tissue-piercing electrode implanted in the basal and/or septal region of the left ventricular myocardium of the patient's heart from the triangle of Koch region of the right atrium through the right atrial endocardium and central fibrous body. The device may include a right atrial electrode, a right atrial motion detector, or both. The device may be implanted completely within the patient's heart or may use one or more leads to implant electrodes in the patient's heart. The device may be used to provide cardiac therapy, including single or multiple chamber pacing, atrioventricular synchronous pacing, asynchronous pacing, triggered pacing, cardiac resynchronization pacing, or tachycardia-related therapy. A separate medical device may be used to provide some functionality for cardiac therapy, such as sensing, pacing, or shock therapy.

Abstract: An electrical stimulation lead includes a lead body having a proximal end and a distal end. A connection interface is coupled to proximal end and a tip electrode is coupled to the distal end. The tip electrode is in electrical communication with the connection interface. A suture line having a barbed structure extends from the tip electrodes. In some examples, the electrical stimulation lead includes a flexible helical electrode capable of engaging tissue. In some examples, the suture line is biodegradable. A method for using an electrical stimulation lead. The method includes placing a tip electrode in a first tissue by pulling the tip electrode into place using a suture line that has a barbed the structure. The method further includes applying electrical stimulation therapy and extracting the tip electrode.

Abstract: An example device includes an elongated housing, a first and second electrode, and signal generation circuitry. The housing can be implanted within a single first chamber of the heart. The first electrode extends distally from the distal end of the elongated housing. A distal end of the first electrode can penetrate into wall tissue of a second chamber of the heart. The second electrode, extending from the distal end of the elongated housing, is configured to flexibly maintain contact with the wall tissue of the first chamber without penetration of the wall tissue of the first chamber by the second electrode. Signal generation circuitry can be within the elongated housing and coupled to the first and second electrode. The signal generation circuitry can deliver cardiac pacing to the second chamber via the first electrode and the first chamber via the second electrode.
