摘要: A method of controlling operation of a medical device includes delivering electrical cardiac pacing energy to a left conduction bundle branch (LBB) area of the subject according to a primary pacing mode that uses a tip electrode of an implantable cardiac lead connected to the medical device for cathodal capture in the LBB area; monitoring for loss of cathodal capture of the LBB area when delivering the electrical cardiac pacing energy in the primary pacing mode; and changing to a backup pacing mode when the loss of cathodal capture is detected, wherein the backup pacing mode uses a ring electrode of the implantable cardiac lead for anodal capture in an interventricular septum of the subject.

摘要: The present disclosure relates generally to pacing of cardiac tissue, and more particularly to adjusting delivery of His bundle or bundle branch pacing in a cardiac pacing system to achieve synchronized ventricular activation. A left bundle branch (LBB) cathode electrode may be implanted a left side of the septum of the patient's heart proximate to the LBB, and a right bundle branch (RBB) cathode electrode may be implanted on a right side of the septum of the patient's heart proximate to the RBB. One or both cathode electrodes may be used to deliver synchronized left and right bundle-branch pacing based on one or both of an atrial event and a ventricular event. A device for bundle branch pacing may be implanted based on determining whether an LBB block pattern or an RBB block pattern is present in monitored electrical activity.

摘要: Implantable medical systems enter an exposure mode of operation, either manually via a down linked programming instruction or by automatic detection by the implantable system of exposure to a magnetic disturbance. A controller then determines the appropriate exposure mode by considering various pieces of information including the device type including whether the device has defibrillation capability, pre-exposure mode of therapy including which chambers have been paced, and pre-exposure cardiac activity that is either intrinsic or paced rates. Additional considerations may include determining whether a sensed rate during the exposure mode is physiologic or artificially produced by the magnetic disturbance. When the sensed rate is physiologic, then the controller uses the sensed rate to trigger pacing and otherwise uses asynchronous pacing at a fixed rate.

摘要: VfA cardiac therapy uses an implantable medical device or system. The implantable medical device includes a tissue-piercing electrode implanted in the basal and/or septal region of the left ventricular myocardium of the patient's heart from the triangle of Koch region of the right atrium through the right atrial endocardium and central fibrous body. The device may include a right atrial electrode, a right atrial motion detector, or both. The device may be implanted completely within the patient's heart or may use one or more leads to implant electrodes in the patient's heart. The device may be used to provide cardiac therapy, including single or multiple chamber pacing, atrioventricular synchronous pacing, asynchronous pacing, triggered pacing, cardiac resynchronization pacing, or tachycardia-related therapy. A separate medical device may be used to provide some functionality for cardiac therapy, such as sensing, pacing, or shock therapy.

摘要: An intracardiac ventricular pacemaker is configured to operate in in a selected one of an atrial-tracking ventricular pacing mode and a non-atrial tracking ventricular pacing mode. A control circuit of the pacemaker determines at least one motion signal metric from the motion signal, compares the at least one motion signal metric to pacing mode switching criteria, and, responsive to the pacing mode switching criteria being satisfied, switches from the selected one of the non-atrial tracking pacing mode and the atrial tracking pacing mode to the other one of the non-atrial tracking pacing mode and the atrial tracking pacing mode for controlling ventricular pacing pulses delivered by the pacemaker.

摘要: An implantable medical device system operates in an atrial synchronized ventricular pacing mode and switches to an atrial-asynchronous pacing mode when pacing mode switching criteria are met. A control circuit of the system detects a cycle length change between two atrial cycle lengths determined from a cardiac signal that includes far-field atrial events. If the cycle length change is greater than a change, threshold the control circuit determines if the pacing mode switching criteria are satisfied subsequent to detecting the cycle length change.

摘要: An intracardiac pacemaker system is configured to produce physiological atrial event signals by a sensing circuit of a ventricular intracardiac pacemaker and select a first atrial event input as the physiological atrial event signals. The ventricular intracardiac pacemaker detects atrial events from the selected first atrial event input, determines if input switching criteria are met, and switches from the first atrial event input to a second atrial event input in response to the input switching criteria being met. The second atrial event input includes broadcast atrial event signals produced by a second implantable medical device and received by the ventricular intracardiac pacemaker.
