摘要:
An endoluminal stent graft deployment system incorporating an anchor wire system for temporary anchoring of a stent graft on a delivery device. The anchor wire system has an anchor wire extending from the distal end of the device and releasably retained to the portion of the stent graft by a releasable retention system. The releasable retention system includes a bead on the proximal end of the anchor wire and an anchor release wire which has fastening arrangement to retain the release wire distally of the bead. The anchor release wire extends to a distal end of the deployment system so that the anchor wire release wire can be withdrawn thereby releasing the anchor wire.

A fenestration (32) for a stent graft (30). The fenestration is an aperture in the biocompatible graft material and has at least one flap (38, 40) of a biocompatible graft material covering the aperture on the inside whereby the flap closes off the aperture but can be displaced to allow access through the fenestration. An array of such fenestrations may be placed on a stent graft to facilitate alignment of a branch vessel with a fenestration. A slip knot (46, 46) which can be released by forcing a dilator between the flaps can be used to hold the flaps together.
摘要:
A reinforcing ring (10) for a fenestration (32) of a stent graft which can be surface treated such as by passivation and/or electropolishing. The reinforcing ring has several turns of a substantially inextensible resilient wire (12) in a circular two dimensional planar shape and terminal ends (14) at each end of the wire. The terminal ends each comprising a loop (14) and a tail (16). The tail is folded back and extends around the circular shape. Each of the tails of the terminal loops can have an enlarged end. The reinforcing ring can be straightened out for surface treatment such as passivation and/or electropolishing with substantially no part of the circular shape, the loops or tails touching each other.

A stent graft (2) for treatment of an emergency rupture of, for instance, the aorta adjacent the aortic bifurcation. The stent graft comprising a tubular body (8) with a bifurcation (10) in the tubular body defining a first long leg (12) and a second short leg (14). The second leg has a valve arrangement (18) to prevent fluid flow through the second leg from the stent graft. The valve can be opened from external of the stent graft for the placement of a leg extension stent graft (90) therethrough.

A prosthesis deployment device having a elongate catheter body and a trigger wire guide concentric about the elongate catheter body where the trigger wire guide comprises three trigger wire lumens separate and apart from a guide wire lumen, with three trigger wires being at least partially disposed in the three trigger wire lumens and configured to engage a portion of a prosthesis to retain the prosthesis adjacent to the trigger wire guide.

A reinforcing and marker ring for a stent graft is disclosed. The reinforcing and marker ring comprises a plurality of turns of a substantially inextensible resilient wire in a circular shape and terminal ends at each end of the wire. The terminal ends each comprise a loop, each loop attachable to a stent graft having an opening or a fenestration so as to substantially lock a peripheral length of the circular shape. A marker winding is wound helically around the reinforcing wire, the marker winding being viewable on an image display system employing electromagnetic radiation so as to indicate the location of a periphery of the fenestration. The circular shape of the resilient wire, with the marker winding wound around it, is collapsible under radial pressure to form a squashed circular shape for loading into a delivery device, the squashed circular shape self-expandable back to a substantially circular shape upon release from the delivery device. The marker winding defines a curved passageway around the resilient wire of the reinforcing ring, the curved passageway having an internal diameter D, wherein the marker winding is helically wound with a pitch providing at least one winding per length D along the reinforcing wire of the reinforcing ring. The marker winding may be a radiopaque gold wire wound around a nitinol reinforcing wire.

A stent assembly (42) adapted for endoluminal placement by endovascular deployment for the treatment of a false lumen (10) associated with a vascular dissection. The stent assembly has a number of self expanding stents (35) connected together to define an elongate substantially cylindrical lumen wall engaging surface. The stents are adapted to provided pressure on the wall of the lumen adjacent to and extending away from a rupture. A deployment device (40) for the stent assembly includes a sheath (48) and a retention and release arrangement (50) to retain the proximal end (37) of the stent graft to the deployment device. Release of the stent assembly is by withdrawal of the sheath before release of its proximal end by the use of a trigger wire (54) of the retention and release arrangement.

A stent graft (10) has a tubular body (12) of a biocompatible material and at least one fenestration (18, 20). The or each fenestration has a tubular side branch stitched into it with a portion of the tubular side branch extending within the tubular body and a portion of the tubular side branch extending exteriorly of the tubular body. The stitching by which the tubular side branch is mounted into the fenestration extends circumferentially and diagonally from one end of the tubular side branch to the other such that the tubular side branch extends from the tubular body at an angle thereto. The tubular side branch has a resilient ring around at least one and preferably both ends thereof and a self expanding stent in between them. The tubular side branch can have a circumferential linear portion (46) at an inner end to engage against the inner wall (58) of the tubular body. The tubular side branch can have a stent made from a single wire forming a first ring (45), a second ring (47), the second ring defining a plane which is substantially parallel to and spaced axially apart from the plane of the first ring and a number of helical wire portions (49) extending between the first and second ring. Preferably the wire is a shape memory wire such as Nitinol™ wire.

A stent graft delivery arrangement for a stent graft (50) has a proximal end to be deployed into a patient in use and a distal end to remain outside the patient. The stent graft (50) is a tubular body of a biocompatible graft material with a plurality of self expanding stents (54). The stent graft is releasably retained onto the delivery device (132) towards the proximal end thereof. A curve forming arrangement (56) on the stent graft is arranged to curve a portion of the stent graft towards its proximal end. A pulling arrangement (58) extends along the delivery device. The pulling arrangement releasably engages the curve forming arrangement such that pulling on the pulling arrangement causes the a curve forming arrangement on the stent graft to form a curve in a portion of the stent graft towards the proximal end. The pulling arrangement can be subsequently released leaving the curve forming apparatus in place on the stent graft and the portion of the stent graft towards the proximal end thereof being laterally curved. The curve forming arrangement can comprise a suture thread. A gripping arrangement engages the suture thread between the second position and the third position to prevent re-extension of the suture thread after retraction thereof. The gripping arrangement can be a slip knot (107).
