摘要： A business form particularly adapted for generalized use, although it may be used in a medical or hospital environment includes in a first embodiment a wristband that is provided with a laminate backing including a tab portion at the opposite end of a free end, with the tab portion having a cinch slot through which the free end is inserted for securing the wristband about a person's appendage. Several self adhering labels are provided inboard of the tab and may be individually separated from the wristband. The labels and the wristband may each be imprinted with identifying indicia such as bar coding. A second embodiment of the invention includes a page sized form with one or more self laminating wristbands readily separable from a carrier with each wristband comprised of a layer of face stock and a laminate, with the wristband including a printable face stock region die cut into the face stock and a strap portion, a laminating portion, and a cinch die cut into the laminate layer. The laminating portion includes two halves which fold together about a fold line to enclose the face stock, with the strap portion extending from one of said halves. The cinch comprises two slots which may both be located outboard of the face stock, or on both sides of the face stock. The free end of the strap may be adhered in place by folding over one of the slots, or by adhering a patch of adhesive carried at the tip of the free end. The face stock may be extended to the edge of the cinch slot to shield the strap from contacting adhesive. Any of the wristband constructions may be provided on a page sized sheet along with a plurality of self adhering labels, in any of a number of configurations, to suit any particular application, as desired by a user.

摘要： A business form particularly adapted for use in a medical or hospital environment includes in a first embodiment a wristband assembly that is readily separable from a carrier comprised of a layer of face stock and a laminate, with the wristband including a printable face stock region die cut into the face stock and a strap portion, a laminating portion, and a cinch die cut into the laminate layer. The laminating portion includes two halves which fold together about a fold line to enclose the face stock, with the strap portion extending from one of said halves. The cinch may be located in one of two locations, either in an extension from the other side of the face stock or intermediate the face stock and the strap portion. In either case the cinch comprises a slot through which the strap portion is inserted and then adhered to itself after its length has been adjusted by the medical professional. If an extension is provided, it may be folded over to adhere to the strap portion and clamp it in place there as well as being adhered at its end back onto itself, as described. An extender is also described which includes a clamshell joinder and a tail portion. The clamshell portion may be applied to the strap portion anywhere along its length to extend the effective length of the strap portion. Any of the wristband constructions may be provided on a page sized sheet along with a plurality of self adhering labels, in any of a number of configurations, to suit any particular application, as desired by a user.

The imprintable portion has a width equal to the width of the tape and the elongated and stubby portions each have a width less than that of the tape. The elongated strap portion has a longitudinal edge aligned with one of the side edges of the tape and an opposite longitudinal edge defined by one of the lines of weakness. The stubby strap portion has a longitudinal edge aligned with an opposite side edge of the tape relative to the elongated strap portion and an opposite longitudinal edge defined by another one of the lines of weakness.

摘要： The present invention relates to a pressure sensitive laminate that is used in the preservation of confidential patient information. More specifically, the label of the present invention employs one or more coatings of substantially opaque materials on a substrate such that information provided on a pre-existing patient label or identification area can be readily concealed by the label and the information is not visible from the reverse side of the structure to which the over label has been applied. The label construction further includes a destructible substrate that may use a series of discontinuous and inter operable slits along with a permanent pressure sensitive adhesive to prevent the ready removal of the over label from the surface or article to which it has been applied.

摘要： An information band for mounting an electrically conductive circuit, such as a radio-frequency identification (RFID) tag. The band comprises a base strip and a top strip. A single sheet of label stock can include a plurality of such bands such that the sheet can be passed through a conventional printer to accept printed indicia onto a surface of one or more bands. These bands can be die cut to facilitate individual removal. The top strip can be either integral with the base strip or a separate piece that can be affixed to a surface of the base strip. Once indicia has been printed to the band, the RFID tag may be manually affixed between the base strip and the top strip, where adhesive disposed therebetween secures the RFID tag. The adhesive may be peripherally deposited such that an adhesive-free zone is formed between the two layers so that when the RFID tag is attached, it does not come in contact with the adhesive.

In other words, the laminating portion includes two halves which fold together about a fold line to enclose the face stock as before, except that the halves fold over end-to-end. The cinch comprises two slots which may both be located outboard of the face stock, and in the laminate portion alone or in both the face stock and laminate portion. Any of the wristband constructions may be provided in any combination, with or without labels, on a sheetlet or a page sized sheet, in any of a number of configurations, to suit any particular application, as desired by a user.
