    摘要: A peripheral memory patch apparatus for attachment to a patient's skin includes a high capacity memory for storing physiologic data uplinked from an implantable medical device. A resilient substrate provides support for a memory, microprocessor, receiver, and other electronic components. The substrate flexes in a complimentary manner in response to a patient's body movements. The substrate is affixed to the patient's skin with the use of an adhesive which provides for comfort and wearability. The low profile peripheral patch apparatus is preferably similar in size and shape to a standard bandage, and may be attached to the patient's skin in an inconspicuous location. A status indicator provides for a visual, verbal, or tactile indication of the operational status of the peripheral memory patch. Uplinking of physiologic telemetry data from the internal memory of an implantable medical device to the peripheral memory patch is initiated in response to a transfer signal produced by the peripheral memory patch. The transfer signal may be generated by the implantable medical device or upon actuation of a switch by the patient. Various telemetry techniques including radio frequency, acoustic, and body bus telemetry techniques, may be employed to transfer information between the implantable medical device and the peripheral memory patch.

In response to detecting or measuring such a signal which meets certain amplitude, frequency and/or phase characteristics, the data monitor directs or controls voltage supply circuits to increase or decrease the voltages provided to certain circuits within the medical device. The voltages so employed are tailored to reduce the amount of power consumed by the medical device while preserving computational performance.

The digital signal processor, microcontroller or microprocessor can be operated at lower clock frequencies relative to those that would be required by one of such processors to complete the multiple functions within a predetermined time period but having no pipeline architecture. With reduced clock frequency, power consumption is reduced. Further, with reduced clock speed, supply voltages applied to such processors may also be reduced.

    摘要: A system for communicating with a medical device implanted in an ambulatory patient and for locating the patient in order to selectively monitor device function, alter device operating parameters and modes and provide emergency assistance to and communications with a patient. The implanted device includes a telemetry transceiver for communicating data and operating instructions between the implanted device and an external patient communications control device that is either worn by or located in proximity to the patient within the implanted device tranceiving range. The control device preferably includes a communication link with a remote medical support network, a global positioning satellite receiver for receiving positioning data identifying the global position of the control device, and a patient activated link for permitting patient initiated personal communication with the medical support network. A system controller in the control device controls data and voice communications for selectively transmitting patient initiated personal communications and global positioning data to the medical support network, for initiating telemetry out of data and operating commands from the implanted device and transmission of the same to the medical support network, and for receiving and initiating re-programming of the implanted device operating modes and parameters in response to instructions received from the medical support network. The communications link between the medical support network and the patient communications control device may comprise a world wide satellite network, hard-wired telephone network, a cellular telephone network or other personal communications system. Methods and apparatae are also described that enhance the ability of the medical system to find patients and to get reports on patient and medical device status, and even update medical device programming using such facilities, and others described in detail within.

Each processor of the multiple systems performs at least one particular function in a predetermined time period. The multiple digital signal processors of such systems can be operated at lower clock frequencies relative to those that would be required by one of such processors to complete the multiple functions within the predetermined time period. With reduced clock frequency, power consumption is reduced. Further, with reduced clock speed, supply voltages applied to such digital signal processors may also be reduced.

    摘要: A multi-chamber cardiac pacing systems for providing synchronous pacing to at least the two upper heart chambers or the two lower heart chambers or to three heart chambers or to all four heart chambers employing one or more field density clamp (FDC) sense amplifiers for accurately sensing and timing cardiac depolarizations of the right and left heart chambers is disclosed. The synchronous pacing of one of the right and left heart chambers is provided on demand following expiration of programmable pace CDW and sense CDW that are started by both a paced event and a sensed event first occurring in the other of the right and left heart chambers. The delivery of the pacing pulse is inhibited by a sensed event detected in the other of the right and left heart chambers before the expiration of the corresponding CDW. In a four channel atrial and ventricular pacing system, the right and left atrial chambers are sensed and paced as necessary upon at the end of a V-A escape interval and right and left, pace and sense, AV delays are commenced for sensing ventricular depolarizations in the right and left ventricles. The four channel system is programmable to pace and sense in three selected heart chambers. Each FDC sense amplifier allows the timing of a short CDW from a paced event or a sensed event. Preferably, a pacing output stage is coupled with the FDC sense amplifiers to deliver pacing pulses to each heart chamber.

    摘要: A cardiac pacemaker with an analog telemetry system. A calibration circuit within the pacemaker is adapted to provide a reference signal of known character to the pacemaker's telemetry system. The reference signal is transmitted across the telemetry link as if it were an actual cardiac signal, and received by an external programmer. Since the reference signal has known, predetermined qualities, the programmer can automatically calibrate and scale the telemetry signal from the pacemaker, thereby increasing the accuracy of the telemetry channel. The increased accuracy is particular useful in assessing rejection of a transplanted heart, which is known to be associated with a 15% decline in the peak R-wave amplitude of the cardiac signal.
