An apparatus and method for determining a volume of tissue mass destroyed. The present invention includes a temperature probe and a laser probe having a temperature sensor. The laser probe and temperature probe are inserted to measure a temperature of the tissue mass and a temperature of tissue mass surrounding the tissue mass. By determining the volume of tissue mass destroyed, a graphical representation of the volume of tissue mass destroyed is provided whereby real-time visual monitoring of the destruction of the tissue mass is achieved.

An interstitial laser therapy control system is disclosed. The control system includes a thermistor controller apparatus, a microprocessor, a storage device, at least one input device, a display, an electro-mechanical shutter switch, an electro-mechanical emergency shutoff switch, and an electro-mechanical master power switch. The microprocessor is configured to monitor temperatures detected by one or more thermistors connected to the thermistor controller. Based on the temperatures, the microprocessor determines when treatment has been successful and when application of laser energy needs to be halted. The control system also enables an operator to pause and resume treatment using the input device. The electro-mechanical shutter switch sends a signal to a laser source to close a shutter on the laser. The electro-mechanical emergency stop button causes power to be cut off to the laser source. The electro-mechanical master power switch shuts off power to all components in the interstitial laser therapy apparatus.

The apparatus employs a combination of a mammography unit, an interstitial laser treatment device attached to the mammography unit and a treatment platform positioned relative to the mammography unit to enable the interstitial laser therapy to be performed. The use of the treatment platform with the mammography unit enables the interstitial laser therapy to be performed and, if necessary, adjunctive therapy to be performed in the same treatment room without transferring the patient to a new platform.

Improved apparatus for carrying out minimally invasive interstitial laser hyperthermia to induce coagulation necrosis in tissue masses located deeply within the body of a subject. Among examples are malignant tumors in the liver or breast. Low power laser radiation is introduced through an optical fiber extending through a cannula or probe guided into place by medical imaging techniques and inserted directly into the target tissue mass. A thermocouple is secured to the outside of the cannula to sense the temperature of the target tissue irradiated by the laser energy. The thermocouple is located slightly behind the distal end of the cannula and the entire probe assembly is gold plated enabling it to respond primarily to heat transmitted by conduction from the tissue and not by radiation from the laser. In one operating mode described, after the probe is inserted into the tumor, the probe is withdrawn while laser energy is administered at a rate sufficient to maintain the temperature at the thermocouple within a predetermined therapeutic range. In one motorized embodiment, a motor withdraws the cannula at a predetermined speed while the laser is firing, to maintain an effectively high uniform tissue temperature. In another automatic embodiment, the motor speed is controlled in response to the tissue temperature sensed by the thermocouple on the outside of the cannula. This leaves a cylindrical core of necrotic tumor tissue in the wake of the probe as it is drawn through the target tissue.

Apparatus and method for locally generating hyperthermia-induced coagulation necrosis in tissue masses located deeply within the body of a subject. High energy laser radiation is introduced into the target tissue mass using an optical fiber inserted into a metallic cannula through which a suitable fluid is coaxially flowed. A combination of the fluid flow and the slow withdrawal of the cannula/fiber combination during therapy protects the optical fiber tip, thereby preserving its energy transmission characteristics. After establishment of a small fluid bolus in front of the fiber tip, fluid flow which is approximately proportionate to the laser energy employed has been found to be most effective in this regard.
