Abstract: Systems and methods are disclosed to control transition of a stimulation circuit configured to generate first and second stimulation signals in different respective first and second stimulation modes between the different respective first and second stimulation modes using a patient metric determined using received physiologic information of the patient including at least one of thoracic impedance information or respiration information from a first time period.

Abstract: Systems and methods are disclosed to optimize resources of a medical device system, including to analyze physiologic information of a patient occurring over specific time periods relative to a prior heart failure event, to adjust, in response to a detected or received trigger event, a baseline value for the patient from a long-term baseline value to one of a shorter-term baseline value or a static value indicative of a baseline value at a prior time relative to the detected or received trigger event, to determine a readmission score for the patient based on the analyzed physiologic information, and to provide a control signal to control a mode or operation of the medical device system based on the readmission score to optimize resources of the medical device system.

Abstract: Systems and methods are disclosed to determine an indication of a predicted response of a patient to at least one of cardiac resynchronization therapy (CRT) or multi-site pacing (MSP) therapy based on a patient metric determined using received physiologic information of the patient including at least one of heart sound information or respiration information from a first time period prior to the CRT or MSP therapy.
