    摘要: This is a combination method and system for intravascularly accessing and visualizing a body structure. A system is disclosed comprising a sheath, a balloon catheter, and a catheter deflection mechanism. A micro-deflection mechanism, which may be placed into the lumen of devices such as cardiac leads or electrophysiology catheters, is also provided for accurate guidance and placement of such devices to their target location. In one embodiment, a procedure for a lead or catheter placement is described in which a combination deflection mechanism, balloon catheter, and sheath assembly access the coronary sinus ostium via a superior approach. A distal balloon on the catheter is inflated and an occlusive venogram is obtained under fluoroscopic guidance to visualize the coronary veins. The sheath is advanced into the coronary sinus ostium and the balloon is deflated. The catheter and deflection mechanism are then removed. Next, a micro-deflection mechanism that has been inserted into the lumen of a cardiac lead is deployed through the sheath, allowing the lead to deflect with a high degree of accuracy and control as it is placed in the selected location.

    摘要: An enclosed connection system for connecting first and second elongate electrical conductors includes a transverse wedge connector and an enclosure. The transverse wedge connector includes an electrically conductive first connector member, an electrically conductive second connector member, and a clamping mechanism. The first connector member includes: a first body having inner and outer opposed ends; a first hook portion on the outer end of the first body, the first hook portion defining a first channel to receive the first conductor; and a first abutment portion on the inner end of the first body. The second connector member includes: a second body having inner and outer opposed ends; a second hook portion on the outer end of the second body, the second hook portion defining a second channel to receive the second conductor; and a second abutment portion on the inner end of the second body. The clamping mechanism is selectively operable to displace the first and second connector members relative to one another from an open position to a clamping position to clamp the first conductor in the first channel and between the first hook portion and the second abutment portion and to clamp the second conductor in the second channel and between the second hook portion and the first abutment portion to thereby form a connection.     摘要: This is a tool used during open heart or open chest surgery for creating lesions or lines of ablation in tissue to treat atrial fibrillation. It is a relatively short device having a rigid or semi-rigid shaft connected on one end to a handle and on the other end to a more flexible distal tip having one or more electrodes. The distal end of the shaft is angled so to define a first plane of deflection. A deflection mechanism consisting of a pull wire and flat spring housed in a lumen of the tool is manipulable via the handle for deflecting the distal tip in a plane different than the first plane of deflection, allowing direct contact with the curved surface of the tissue, either epicardial or endocardial, to be ablated. The tool may contain fiber optic or other data lines for transmitting data relevant to the patient, and it may be irrigated to cool the tool and the electrodes or to deliver therapeutic fluids to tissue. The tool may also contain temperature sensing device such as a thermocouple or thermistor, which can optionally be configured in a feedback and control system with the irrigation to selectively monitor and control the temperature of the tool or surrounding tissue.
