Surgical devices and methods are described herein that provide improved motor control and feedback, thereby combining advantages of manually-operated and powered surgical devices. In one embodiment, a surgical device includes a proximal handle portion (10) that includes a motor, a distal end effector (14) coupled to the handle portion, and a cutting element configured to cut tissue engaged by the end effector (14), wherein the motor is configured to supply power that moves the cutting element. The device also includes a motor control mechanism configured to cause the amount of the power to dynamically change in response to a manual user input when the cutting element is moving.

Improved tissue fasteners that can be inserted into tissue structures underlying a tissue surface. First and second legs of the fastener may be configured to be advanced distally through a tissue surface and into the tissue. An elongate anvil body may protrude from a fastener support disposed along a base. The anvil body may have a sharpened penetrating end. The first leg can be driven through a desired location on the surface of the tissue and against a receptacle of the anvil body so as to deform the fastener and affix it to the tissue. A tissue-engaging surface may be movable relative to the fastener support, with the tissue-engaging surface helping to coordinate the distal movement of the fastener and positioning of the anvil within the tissue via an articulated linkage so that a correlation is maintained between leg penetration depth and deformation of the leg or legs.

A handle (5) for actuating surgical components (45), primarily used in cardiovascular or arthroscopic surgery is a single-piece handle, including an outer portion (10) and an inner portion (15). The inner portion is substantially within the outer portion, and includes an actuating component (50), and a guiding component (55) connected to the actuating component. The actuating component includes a finger grip (65), and a spring force component (70) which biases the finger grip and the guiding component to extend towards a distal tip (35) of the neck (30). A stress limiting component (110) limits stress at a critical point of the spring force component by limiting movement of the spring force component at that point during actuation of the actuating component. The guiding component is adapted for movement along a longitudinal axis of the handle, in response to actuation and release of the actuating component, allowing a surgical component connected to a distal tip (80) of the guiding component to function.

Hand-held instruments have an actuator assembly which may be employed to actuate a distally mounted tool by application of radial force about the entirety of the instrument circumference (i.e., is omni-actuatable). Most preferably, the actuator assembly (10-1) includes a plurality of circumferentially spaced-apart generally L-shaped actuator levers (20) defining respective arcuate bearing surfaces (22-1) along exterior edge regions thereof, and a retaining ring (22) which circumferentially bounds the actuator levers around the exterior edge regions thereof. The retaining ring most preferably defines an interior stationary arcuate guide surface (20-3) in conformable mated relationship to the bearing surfaces (22-1) of said actuator levers. Radial force applied to the proximally extending arm sections of the levers thereby responsively causes the bearing surfaces thereof to be slideably moved along the guide surface of said retaining ring so as to translate such radial force into longitudinal movements of an elongate actuation pin (18) between retracted and extended positions thereof. By providing a tool which operatively coacts with the actuation pin, a surgeon may actuate the tool as needed by applying radial force at any point about the circumference of the actuator assembly.

The body is configured to receive an ultrasonic transducer for selectively generating an oscillation at a first or a second predetermined power level. The shaft assembly projects from the body and includes an acoustic waveguide connected to the ultrasonic blade. The actuation assembly includes a selector collar generally surrounding the body and a plurality of activation buttons disposed radially about the body proximate to the selector collar. The selector collar is selectively movable along the body between a first position and a second position for selecting between the first and second predetermined power levels. The plurality of activation buttons are configured to direct the ultrasonic blade to oscillate with the selected first or second predetermined power levels.
