    摘要: An electrosurgical medical device for creating thermal welds in engaged tissue that provides general grasping and dissecting functionality. In an exemplary embodiment, at least one jaw of the instrument defines a tissue engagement plane carrying electrosurgical energy delivery means. The jaw assembly, in one mode of operation, can be used for general grasping and dissecting purposes wherein the jaws close in a non-parallel manner so that the distalmost jaw tip only engage tissue with little movement of the actuator lever in the handle of the instrument. In another mode of operation, the jaw assembly closes close in a parallel manner under very high compression to enable tissue welding.

    摘要: An apparatus and method for use in performing ablation or coagulation of organs and other tissue includes a metallized fabric electrode array which is substantially absorbent and/or permeable to moisture and gases such as steam and conformable to the body cavity. The array includes conductive regions separated by insulated regions arranged to produce ablation to a predetermined depth. Following placement of the ablation device into contact with the tissue to be ablated, an RF generator is used to deliver RF energy to the conductive regions and to thereby induce current flow from the electrodes to tissue to be ablated. As the current heats the tissue, moisture (such as steam or liquid) leaves the tissue causing the tissue to dehydrate. Suction may be applied to facilitate moisture removal. The moisture permeability and/or absorbency of the electrode carrying member allows the moisture to leave the ablation site so as to prevent the moisture from providing a path of conductivity for the current.

    摘要: A system and method for detecting perforations in a body cavity. In accordance with the method of the invention, a fluid (liquid or gas) is delivered into a body cavity to slightly pressurize the cavity. A pressure sensing system monitors the pressure within the cavity for a predetermined test period. If cavity pressure is not substantially sustained during the test period, the physician is alerted to further assess the cavity for perforations before initiating treatment within the cavity. In a preferred form of the system, a medical treatment system such as an RF ablation system is provided with perforation detection functionality. The system preferably includes a pre-test lockout that prevents RF power delivery unless a perforation detection procedure previously has been performed.

    摘要: An electrosurgical medical device and technique for creating thermal welds in the transected margin of engaged tissue. The working end of the invention carries a bi-polar electrode arrangement that provides a novel type of Rf current flow for tissue welding described as an subfascial-to-fascial (or medial-to-surface) bi-polar approach. The working end of the device carries an openable and closable jaw assembly with paired first and second jaws for engaging tissue. The paired jaws have an axial channel for receiving a sliding (blade) member for transecting the captured tissue. The paired jaws carry electrode surfaces having a common polarity so that Rf current will not flow directly between the jaws which engage the surface or fascial tissue layers. The transecting member, when fully extended after transecting the captured tissue, carries an elongate electrode of an opposing polarity that engages the medial or subfascial layers of the tissue. Thus, bi-polar current is directed to flow from the transecting member electrode that engages medial tissue layers to both jaws that engage opposing surface tissue layers to create effective current densities in the medial tissue layers, even when transecting thick tissue volumes or vessels with thick non-uniform fascial layers.

    摘要: Multiple lumen tubing is extruded by first extruding an inner tube of molten plastic about a metallic, bore-forming mandrel wire, followed by extruding about the inner tube an outer tube of molten plastic having a lumen formed by air mandrel means, with the inner and outer tubes being bonded together. This may be accomplished by means of a mandrel which is positioned in an extrusion die, with the mandrel defining a distal end portion positioned and shaped to form a lumen in the tubing, and an air flow passage extending through the mandrel to provide an air mandrel to the newly formed lumen to prevent its collapse. The distal end portion of the mandrel may be carried by an intermediate neck portion which has at least one transverse dimension that is less than the corresponding transverse dimension of the distal end portion.
