Abstract: A medical device performs a method for detecting an atrial tachyarrhythmia by determining RR intervals between successive R-waves of a cardiac electrical signal and determining classification factors from the R-waves identified over a predetermined time period by determining at least a first classification factor correlated to variability of the RR intervals and a second classification factor indicating a presence of a ventricular tachyarrhythmia. The device classifies the cardiac electrical signal of the predetermined time period as unclassified, atrial tachyarrhythmia or non-atrial tachyarrhythmia by comparing the determined classification factors to classification criteria. The predetermined time period is classified as unclassified when the second classification factor indicates the presence of a ventricular tachyarrhythmia.

The leads include an elongated lead body having a distal portion and a proximal end, a connector at the proximal end of the lead body, a defibrillation electrode located along the distal portion of the lead body, wherein the defibrillation electrode includes a first electrode segment and a second electrode segment proximal to the first electrode segment by a distance. The leads may include at least one pace/sense electrode, which in some instances, is located between the first defibrillation electrode segment and the second defibrillation electrode segment.

Abstract: An extra-cardiovascular implantable cardioverter defibrillator senses R-waves from a first cardiac electrical signal by a first sensing channel and stores a time segment of a second cardiac electrical signal in response to each sensed R-wave. The ICD determines intervals between successively sensed R-waves and, in response to at least a predetermined number of the intervals being less than a tachyarrhythmia detection interval, analyzes at least a portion of the time segment of the second cardiac electrical signal corresponding to a most recent one of the sensed R-waves to confirm the most recent one of the R-waves. The ICD updates an unconfirmed beat count in response to the most recent one of the R-waves not being confirmed and withholds detection of a tachyarrhythmia episode in response to the unconfirmed beat count being equal to or greater than a rejection threshold.

Abstract: Techniques are described for discriminating SVT and, in particular, rapidly conducting AF. The techniques include detecting an onset of a fast rate of ventricular events sensed from a cardiac electrical signal and detecting a pause in the fast rate of ventricular sensed events. A threshold number of ventricular event intervals required to detect a ventricular tachyarrhythmia is detected with each of the threshold number of ventricular event intervals being less than a tachyarrhythmia detection interval. Detection of the ventricular tachyarrhythmia and an electrical stimulation therapy for treating the ventricular tachyarrhythmia are withheld in response to at least the pause being detected.

Abstract: A medical device is configured to deliver His-Purkinje pacing pulses according to multiple settings of a pacing control parameter and determine an electromechanical time delay from a ventricular electrical event to a fiducial point of the pressure signal for each of the pacing control parameter settings. The medical device may be configured to select an operating pacing control parameter from the pacing control parameter settings based on a determined electromechanical time delay being less than a threshold interval. The medical device may deliver pacing pulses to the His-Purkinje conduction system according to the selected operating pacing control parameter.

Abstract: An implantable medical device system capable of sensing cardiac electrical signals includes a sensing circuit, a therapy delivery circuit and a control circuit. The sensing circuit is configured to receive a cardiac electrical signal and sense a cardiac event in response to the signal crossing a cardiac event sensing threshold. The therapy delivery circuit is configured to deliver an electrical stimulation therapy to a patient's heart via the electrodes coupled to the implantable medical device. The control circuit is configured to control the sensing circuit to set a starting value of the cardiac event sensing threshold and hold the starting value constant for a sense delay interval. The control circuit is further configured to detect an arrhythmia based on cardiac events sensed by the sensing circuit and control the therapy delivery circuit to deliver the electrical stimulation therapy in response to detecting the arrhythmia.

Abstract: An implantable medical device performs a method that includes detecting a cardiac event interval that is greater than a P-wave oversensing threshold interval. In response to detecting the cardiac event interval greater than the P-wave oversensing threshold interval, the device determines the amplitude of the sensed cardiac signal and withholds restarting a pacing interval in response to the amplitude satisfying P-wave oversensing criteria. A pacing pulse may be generated in response to the pacing interval expiring without sensing an intrinsic cardiac electrical event that is not detected as a P-wave oversensing event.

Abstract: A medical device system, such as an extra-cardiovascular implantable cardioverter defibrillator ICD, senses R-waves from a first cardiac electrical signal by a first sensing channel and stores a time segment of a second cardiac electrical signal in response to each sensed R-wave. The medical device system determines a morphology parameter correlated to signal noise from time segments of the second cardiac electrical signal, detects a noisy signal segment based on the signal morphology parameter; and withholds detection of a tachyarrhythmia episode in response to detecting a threshold number of noisy signal segments.

Abstract: A medical device is configured to deliver an electrical stimulation pulse to a heart of a patient, determine a pre-stimulation cardiac event amplitude prior to delivering the electrical stimulation pulse and adjust a cardiac event sensing threshold according to a first post-stimulation decay sequence in response to the electrical stimulation pulse delivery.
