Typically, the power module is alternately operable in both a controlled current mode and a controlled voltage mode.

The source of the ECG signals may be the defibrillation electrodes. The defibrillation circuit may be contained in the control box.

A charge storage device located externally of a patient's body is charged, and is discharged through the patient's body through at least two discharge electrodes connected by electrical circuitry to opposite poles of the charge storage device. A continuous discharge of the charge storage device through the electrodes is controlled so as to produce at least one phase of a current waveform that includes a ripple. The ripple has a height less than one-third of the height of the peak current of the phase, and the difference between the peak current of the phase and the lowest current of the phase is less than one third of the peak current of the phase. The current waveform has a sensing pulse portion that is integral with the therapeutic discharge portion of the current waveform.

An advisory algorithm automatically determines whether a synchronized shock should be delivered (e.g., because a shockable tachycardia is detected) and what ECG event should trigger delivery of the shock. And following initiation of a shock by the operator, the defibrillator automatically waits until the required ECG event is detected to deliver the shock. This happens automatically, without the operator being asked to decide between a synchronized and unsynchronized shock, or having to do anything differently to deliver a synchronized shock (e.g., without having to hold a button depressed as necessary to deliver a synchronized shock in manual defibrillators). The unit delivers a shock at the end of a predetermined period if the required ECG event has not been detected, so that a shock always results when the operator initiates one.

A defibrillator with an alarm driven by both a heart rate detector and a fibrillation/tachycardia advisory algorithm. The defibrillator is a semiautomatic. By driving the alarm from both sources, the operator is provided with more information; instead of simply being advised of the onset of fibrillation or shockable tachycardia, the operator of a semiautomatic defibrillator is now also advised of the onset of asystole, bradycardia, and nonshockable tachycardia (e.g., superventricular tachycardia). The advisory algorithm can often provide a more timely indication of the onset of an abnormal rhythm than can the conventional heart rate detector, which relies on averaging over multiple heart beats, and thus is slow to detect a change in waveform. And, the advisory algorithm will sometimes detect an abnormal ECG that would have gone undetected by a conventional heart rate alarm. The defibrillator can be combined with an external pacemaker, so that both defibrillation and pacing are available to respond to the conditions detected by the improved alarm. For example, if asystole or bradycardia is detected, the unit can provide pacing stimuli rather than defibrillation stimuli.

The control circuitry 32 is configured so that when the switches 116, 118 are both closed a defibrillation pulse is initiated. Each of the switches is connected in parallel with a corresponding circuit element 124, 126, each having a characteristic impedance, and being shorted out when the corresponding switch 116, 118 is closed. The invention provides for measuring an indication of the series impedance of the discharge control circuitry 32 to determine the open or closed state of the switches by comparing a measured impedance value indication with an impedance value indication calculated for a given open or closed state of the switches.
