摘要: A unitary flexible sheet is folded to define: (i) a panel, defining a plane, and having a first side facing in a first direction away from the plane, (ii) a first tab, disposed on the first side of the panel, and protruding in the first direction away from the panel, and (iii) a second tab, disposed on the first side of the panel, and protruding in the first direction away from the panel. Between the first tab and the second tab are secured (i) a first-leaflet commissural portion of a first prosthetic leaflet, and (ii) a second-leaflet commissural portion of a second prosthetic leaflet. At the first tab, the sheet is folded to define a first cushion at a downstream edge of the first tab. At the second tab, the sheet is folded to define a second cushion at a downstream edge of the second tab.

In one implementation, an expandable annular valve body may include a plurality of atrial anchoring arms and ventricular anchoring legs extending radially outward therefrom. At least part of the flexible portion may be configured for placement at a location radially outward from an outer diameter defined by terminal ends of the ventricular anchoring legs.

A method comprises (A) sandwiching, between a first tab portion and a second tab portion, (i) a first commissural portion of a first leaflet, and (ii) a second commissural portion of a second leaflet; (B) attaching the first tab portion and the second tab portion to the sheet by stitching a first stitching through the tab portions and the commissural portions; (C) subsequently, covering the first stitching by: (i) forming the first tab portion into a first tab having an outer layer and an inner layer, and (ii) forming the second tab portion into a second tab having an outer layer and an inner layer; and (D) subsequently, stitching a second stitching through the outer and inner layers, and the commissural portions. Other embodiments are also described.

摘要: Apparatus is provided for use at a native valve of a heart of a subject. The apparatus includes a delivery tube, transluminally advanceable to the heart; and a prosthetic valve support, configured to support a prosthetic valve at the native valve, and comprising an upstream support portion. The upstream support portion has a working configuration in which it is generally annular and has (1) a tissue-contacting side configured to be placed against an atrial surface of the native valve, and (2) an opposing side, and defines an opening therebetween. The upstream support portion also has a delivery configuration in which the upstream support portion is rolled up to define a channel, and is disposed within the delivery tube. The upstream support portion is configured to be transitioned from the delivery configuration to the working configuration by being exposed from the delivery tube, and unrolled.

The controller includes a rod (442) that extends from a proximal part of the tool to the housing; and an actuator (444). The actuator is fixedly coupled to the rod, includes an engaging element (446) that engages the track, and is rotatable with respect to the housing. The controller and the housing mechanically cooperate such that rotation of the actuator with respect to the housing slides the housing longitudinally with respect to the actuator. 摘要: A tool is configured for transluminal delivery of an implant. The tool has a proximal part and a distal part, and comprises a shaft, a nosepiece, a sheath, and a balloon. The nosepiece is fixed to the shaft, and is arranged with respect to the implant such that the implant extends proximally away from the nosepiece and over the shaft. The sheath houses the implant. The balloon is disposed at the distal part of the tool, and is in fluid communication with the proximal part of the tool. When maximally inflated, the balloon has (i) a widest part that has an inflated diameter that is less than the diameter of the sheath or at most 10 percent greater than the diameter of the sheath, and (ii) a tapered portion that tapers longitudinally away from the widest part and from the nosepiece.
