    摘要: A method and assembly is provided for linking an additional vertebral construct to an existing spinal construct having at least two bone screws engaged to corresponding vertebrae and connected by a connecting member. The assembly includes a fastener configured to engage a bone screw of the existing construct and to engage a connecting member connected to the bone screw. The assembly further includes a linking member having an elongated portion configured to connect to a bone screw of the additional construct and a mounting structure configured for mounting on the bone screw of the existing construct. A locking member fixes the orientation of the linking member relative to the bone screw and a clamping member locks the linking member and existing connecting member to the bone screw of the existing construct.

    摘要: A spinal fixation system includes an elongated rod sized to extend between at least two vertebrae and a number of anchor devices for anchoring the rod to the spine. The anchor devices include a bone engaging fastener having a head defining a spherical socket. A ball insert is configured to be placed within the socket and rotated so that a spherical surface of the ball insert is juxtaposed with the spherical surface of the socket. The insert has a first dimension less than the socket opening and a second dimension that upon rotation causes the insert to be captively retained in the socket. The anchor device further includes a yoke defining a yoke channel for receiving the rod and including a stem that is engaged to the ball insert when the insert is captured within the socket. A sleeve is disposed between the yoke channel and the fastener head for supporting the rod thereon. A set screw is threaded into the yoke channel for clamping the rod against the sleeve. The set screw further operates to draw the ball insert into clamped engagement within the socket.

The network access device is adapted to assign a physical location identifier to an input port, to authenticate the VoIP telephone, wherein the authentication includes receiving a unique device identifier from the VoIP telephone, and to transmit the location identifier and the unique device identifier to the E911 database management system. The E911 database management system is permitted to store the physical location identifier in association with the unique device identifier.
