摘要： A tissue retractor includes a body having proximal and distal ends. The retractor also includes a retraction device having a head connected to the distal end of the body and defining two opposing openings, a connector movably disposed in the body, and two flexible needles of a shape memory material having a memory shape. The needles are fixedly connected to the connector for traveling through a respective one of the openings. The memory shape of the needles includes a portion with an arcuate shape. A one-handed actuation device is connected to the proximal end of the body and is operatively connected to the connector through the body. Upon actuation of the actuation device, the connector can be moved to selectively extend the needles out of the head and withdraw the needles into the head. Methods for using the tissue retractor, in particular for the treatment of Gastroesophageal Reflux Disease, are also provided.

摘要： A flexible endoscopic clip applier includes a flexible coil with a manual actuator coupled to one end and a jaw assembly coupled to the other end. A store of clips is arranged adjacent to the jaw assembly and a clip pusher is arranged adjacent to the store of clips. The actuator includes a lever for opening and closing the jaws, a knob for rotating the jaw assembly, and a crank for dispensing clips. The knob and the lever are coupled to a single control member which extends through the coil to a joiner where it is joined to a pair of pull wires coupled to the jaws. The crank is coupled to a second control member which is threaded along a distal portion. The threaded portion engages a threaded member near the pusher and is coupled to the pusher such that rotation of the threaded control member by the crank causes the pusher to be moved distally.

In operation, actuation of the actuator assembly causes the control member to actuate the surgical tool, and the first engagement member engages the second engagement member to transmit torque applied to the control member to the end effector assembly.

Tendons within the hollow body maintain the hollow body in the relatively flexible and relatively stiff conditions. A method for variably flexing an insertion device for receiving an instrument, includes providing a hollow body having inner and outer sleeves defining a space therebetween in which tendons are disposed. Suction is applied to create a vacuum in the space for frictionally locking the tendons in place between the sleeves in a relatively stiff condition of the hollow body. The vacuum is relieved to release the tendons in a relatively flexible condition of the hollow body.
