A medical device is configured to receive a cardiac signal and determine a morphology matching score from the cardiac signal and a capture detection morphology template that corresponds to a first type of cardiac pacing capture that includes capture of at least a first portion of the His-Purkinje system. The device is configured to detect a second type of cardiac pacing capture in response to the morphology matching score being less than the first match threshold. The second type of cardiac pacing capture is different than the first type of cardiac pacing capture and may include capture of the ventricular myocardium and/or a second portion of the His-Purkinje system different than the first portion.

The sensing device senses a P-wave attendant to an atrial depolarization of the heart via housing-based electrodes carried by the sensing device when the sensing device is implanted outside the cardiovascular system and sends a trigger signal to the intracardiac pacemaker in response to sensing the P-wave. The intracardiac pacemaker detects the trigger signal and schedules a ventricular pacing pulse in response to the detected trigger signal.

This disclosure provides an extravascular ICD system and method for defibrillating a heart of a patient. The extravascular ICD system includes multiple extravascular electrical stimulation leads or lead segments located in close proximity to one another and having respective defibrillation electrodes. The ICD system utilizes the multiple defibrillation electrodes to form an extravascular electrode vector that may result a reduction in the shock impedance and/or a reduction in the DFT compared to extravascular ICD systems that include only a single extravascular defibrillation electrode. An ICD of the system may, for example, deliver a defibrillation shock using an electrode vector in which a first polarity of the electrode vector is formed by electrically coupling first and second defibrillation electrodes of first and second leads, respectively, to the therapy circuitry and a second polarity of the electrode vector is formed by electrically coupling a housing of the ICD to the therapy circuitry.

A medical device is configured to detect an atrial tachyarrhythmia episode. The device senses a cardiac signal, identifies R-waves in the cardiac signal attendant ventricular depolarizations and determines classification factors from the R-waves identified over a predetermined time period. The device classifies the predetermined time period as one of unclassified, atrial tachyarrhythmia and non-atrial tachyarrhythmia by comparing the determined classification factors to classification criteria. A classification criterion is adjusted from a first classification criterion to a second classification criterion after at least one time period being classified as atrial tachyarrhythmia. An atrial tachyarrhythmia episode is detected by the device in response to at least one subsequent time period being classified as atrial tachyarrhythmia based on the adjusted classification criterion.
