摘要:
An elongate body of a medical electrical lead includes at least one conductor formed into a coil that includes a first portion and a second portion, wherein the first portion extends within an outer insulation sheath and the second portion extends outside the outer insulation sheath to be exposed to an environment external to the lead body as an energy dissipating shunt.

An electrical connector has a compressible portion that expands to accept an inserted lead and then contracts around the lead to provide both an electrical and a spring-like mechanical connection to the lead. The connector may have a fluted pin that collapses around the lead body when a set screw of the connector port is tightened. Alternatively, a middle segment of a pin may have indentations or slots that collapse around an inserted lead body. The connector can be attached to the lead body after the lead body is implanted in the body of the patient. In lead systems in which the lead body is implanted using a guide catheter, attaching the connector to the lead body after implantation makes removal of the guide catheter much less difficult.

A system for implanting a medical electrical lead includes an elongated lead body and a relatively non-compliant lead extension joined to a proximal end of the proximal portion of the lead body. The lead body includes a plurality of connector rings positioned about a proximal portion of the lead body. The lead extension includes a plurality of temporary contact surfaces and a plurality of conductors wherein each conductor joins a temporary contact surface to a corresponding connector ring.

A guiding catheter and method of its use are presented wherein the catheter includes an elongate catheter shaft having a proximal region and a distal region and a length. The shaft defines a distal region and includes a distal tip. The distal region defines an arc of approximately 180 degrees and having a radius of 0.3 inches to 0.6 inches. The distal tip forms a half turn of a left-hand helix having a pitch of 0 inches to 0.4 inches. The unique shape of the distal region allows the distal tip be perpendicularly aligned with the septal wall of the right atrium at the His bundle location when the catheter is advanced therein.

A catheter used to deliver a medical electrical lead to a right atrium of a heart in close proximity to a His bundle. The catheter is adapted such that the distal tip confronts the His bundle generally perpendicularly. The catheter includes a proximal portion and a generally hook-shaped distal portion. The distal portion may include curves that direct the distal tip at an angle of over 180 degrees from the direction of the proximal portion.

An extendable medical lead comprises a lead body and a sheath defining a cavity that encloses a length of the lead body. The length of the lead body enclosed within the sheath may be coiled or otherwise gathered such that when extended, the length of the enclosed section of the lead body is greater than the length of the sheath. The sheath may include a seal to help prevent contaminant entry into the cavity in order to help reduce tissue in growth around the length of the lead body disposed within the sheath. A portion of the length of the lead body enclosed within the sheath exits the cavity through an aperture defined by the seal when a tensile force is applied to the lead body.

Medical electrical leads adapted to be implanted within the body, and particularly such leads having at least one distal electrode affixed at a site of a body organ, particularly the epicardium of the heart, employing a light-activated adhesive fixation, and methods and systems for accessing the site, applying the distal electrode to the site, and activating the light-activated adhesive. The lead is preferably implanted by performing a thoracoscopy of the thorax of the patient to visualize the site of the epicardium through a thoracoscope and to provide a pathway to the site of the epicardium. While viewing the site of the epicardium through the thoracoscope, the electrode head is inserted through the provided pathway to apply the plate against the site of the epicardium, and the light-activated adhesive is exposed to a predetermined bandwidth of light to adhere the plate to the epicardium. An elongated introduction tool is selectively operable to grasp and release the electrode head and to conduct light of the predetermined bandwidth to the electrode plate. The introduction tool preferably has an opaque sidewall enclosing a plurality of optical fibers distributed around the circumference of and within the sidewall of the introduction tool to extend lengthwise to the introduction tool distal end, whereby the plurality of optical fiber distal ends are capable of emitting light of the predetermined frequency in a substantially cylindrical emission pattern. The light-activated adhesive comprises one of a light-activated acrylic or cyanoacrylate adhesive that is activated by ultraviolet light that is activated by ultraviolet light. The lead body is formed with a strain relief proximal to the electrode head for absorbing strain placed on the lead body from being transmitted to the electrode head at the lead body distal end and absorbing the strain induced in the lead body by movement of the body organ, e.g., the contraction and expansion of the heart.

An atrial lead system and a method of employing it to provide single electrode bi-atrial pacing. The system includes an atrial pacing lead having electrode located on a distal portion thereof and a guide catheter having a longitudinal lumen and a lateral aperture open to the longitudinal lumen of the catheter proximal to a distal end of the catheter through which the distal portion of the lead may be advanced. The electrode is preferably an active fixation electrode or is associated with an active fixation device. The system is employed by first advancing the guide catheter to the ostium of a patient's coronary sinus and inserting the distal end of the guide catheter into the coronary sinus such that the lateral aperture of the catheter is located in the patient's right atrium. The atrial pacing lead is then advanced through the lumen of the guide catheter such that the electrode exits the lateral aperture of the guide catheter and the electrode is affixed to the tissue of the atrial septum.
