Abstract: A system includes a radiation source, one or more beam-directing elements, and a controller. The controller is configured to cause the radiation source to emit one or more aiming beams at the beam-directing elements such that the aiming beams are directed, by the beam-directing elements, toward an eye of a patient, to verify that each of the aiming beams is properly directed by the beam-directing elements, and, in response to verifying that each of the aiming beams is properly directed by the beam-directing elements, to treat the eye by causing the radiation source to irradiate one or more target regions of the eye with respective treatment beams. Other embodiments are also described.

Abstract: Transcorneal and fiberoptic laser delivery systems and methods for the treatment of eye diseases wherein energy is delivered by wavelengths transparent to the cornea to effect target tissues in the eye for the control of intraocular pressure in diseases such as glaucoma by delivery systems both external to and within ocular tissues. External delivery may be affected under gonioscopic control. Internal delivery may be controlled endoscopically or fiberoptically, both systems utilizing femtosecond laser energy to excise ocular tissue. The femtosecond light energy is delivered to the target tissues to be treated to effect precisely controlled photodisruption to enable portals for the outflow of aqueous fluid in the case of glaucoma in a manner which minimizes target tissue healing responses, inflammation and scarring.

Abstract: A treatment probe for treating an eye of a patient includes an elongate body that defines a handle and a treatment fiber that is housed within the elongate body. The treatment fiber is configured to deliver treatment light energy to the eye. A contact member is disposed on an end of the elongate body. The contact member has a contact surface for positioning on a surface of the eye, two side edges that are positioned on opposite sides of the contact surface, and a fluid channel. The contact surface conforms to the shape of the eye's sclera and the two side edges are shaped to direct fluid that is present on the surface of the eye toward the fluid channel when the contact member is moved laterally across the surface of the eye. The fluid aggregates within the fluid channel and contacts a distal end of the treatment fiber.

Abstract: An imaging probe comprises a camera or endoscope with an external detector array, in which the probe is sized and shaped for surgical placement in an eye to image the eye from an interior of the eye during treatment. The imaging probe and a treatment probe can be coupled together with a fastener or contained within a housing. The imaging probe and the treatment probe can be sized and shaped to enter the eye through an incision in the cornea and image one or more of the ciliary body band or the scleral spur. The treatment probe may comprise a treatment optical fiber or a surgical placement device to deliver an implant. A processor coupled to the detector can be configured with instructions to identify a location of one or more of the ciliary body band, the scleral spur, Schwalbe's line, or Schlemm's canal from the image.

Abstract: A system (20) comprises a radiation source (48) and a controller (44). The controller is configured to designate multiple target regions (84) on an eye (25) of a patient (22) for irradiation with respective amounts of energy, to cause the radiation source to irradiate at least a first one of the target regions, to identify a change in the eye by processing an image of the eye subsequently to causing the radiation source to irradiate at least the first one of the target regions, and to refrain, in response to identifying the change, from causing the radiation source to irradiate a second one of the target regions, which has not yet been irradiated, with the amount of energy designated for the second one of the target regions. Abstract: A method of treating a subject having glaucoma comprises performing excimer laser trabeculostomy (ELT) on a subject having glaucoma and having previously undergone a failed treatment or a treatment that has been rendered ineffective by progression of the disease. In some examples, the failed treatment is a non-surgical treatment comprising administering medicated eye drops. In some examples, the failed treatment is a laser treatment or surgical treatment, such as a trabeculoplasty, iridotomy, iridectomy, trabeculectomy, trabeculotomy, goniotomy, surgical insertion of a shunt or implant, deep sclerectomy, viscocanalostomy, or a combination thereof.

Abstract: Microsurgical instruments having combined illumination, laser ablation, and viscoelastic injection functions. A surgical instrument includes a probe having a main lumen and a port at a distal end thereof. The probe may further include one or more optical fibers within the main lumen, the optical fibers configured to project laser light and illumination light. Laser light may be emitted from the distal end of the probe for disrupting an ocular tissue, while illumination light may be simultaneously emitted, axially or laterally, to provide enhanced visualization of the intraocular space during tissue disruptance. Upon disrupting the tissue, a viscoelastic fluid may be injected from the port to maintain an integrity of the intraocular space.

Abstract: A method for a minimally invasive, cell-selective laser therapy on the eye. The method, based on a short-pulse laser system, allows for different selective types of therapy on the eye. The method is based on a frequency-doubled, continuously working solid-state laser including a pump source and a control unit. The control unit regulates the pump source such that the solid-state laser emits individual pulses with pulse lengths ranging from 50 ns to continuous, wherein pulse lengths ranging from 50 ns to 50 μs are provided for selective therapies and pulse lengths ranging from 50 μs to continuous are provided for coagulative or stimulating therapies, in particular in the range from 1 ms to 500 ms. The proposed method enables a selective treatment of melanin-containing cells in the different areas of the eye via the targeted control of the pump source.

Abstract: Intraocular pressure in an eye is reduced by delivering a high resolution optical coherence tomography (OCT) beam and a high resolution laser beam through the cornea, and the anterior chamber into the irido-corneal angle along an angled beam path. The OCT beam provides OCT imaging for surgery planning and monitoring, while the laser beam is configured to modify tissue or affect ocular fluid by photo-disruptive interaction. In one implementation, a volume of ocular tissue within an outflow pathway in the irido-corneal angle is modified to create a channel opening in one or more layers of the trabecular meshwork. In another implementation, a volume of fluid in the Schlemm's canal is affected by the laser to bring about a pneumatic expansion of the canal. In either implementation, resistance to aqueous flow through the eye is reduced.
