摘要:
A vascular closure device including a sheath with one or more orifices therein to detect blood flow, indicating that the sheath has entered an artery and the relative position of the sheath within the artery. Thus, the sheath can be moved and positioned relative to the artery with having to completely extract the sheath from the artery after initial penetration.

An apparatus for delivering a closure element into an opening formed in a blood vessel or other body lumen and methods for manufacturing and using same. The apparatus is configured to retain the closure element such that the closure element is disposed substantially within the apparatus. The apparatus also can engage, and position the closure element substantially adjacent to, the blood vessel wall adjacent to the opening. During deployment of the closure element, the apparatus expands the closure element beyond a natural cross-section of the closure element such that the closure element, when deployed, is configured to engage a significant amount of the blood vessel wall and/or tissue. Engaging the blood vessel wall and/or tissue, the closure element is further configured to return to the natural cross-section, thereby drawing the engaged blood vessel wall and/or tissue substantially closed and/or sealed, such that hemostasis within the opening is enhanced.

A tissue approximation device and processes for using the device are provided. The device is an implantable, biodegradable construct (except for hernia repairs) that has attachment points emanating from a supportive backing. The device improves the mechanical phase of wound healing and evenly distributes tension over the contact area between the device and tissue. Processes for using the device include wound closure, vascular anastomoses, soft tissue attachment and soft tissue to bone attachment.

An apparatus for sealing a puncture in a body vessel has a sealing member (10) comprising a flexible tubing (11) having an internal passageway (14). The tubing (11) is provided at an open distal end (15) thereof with an elastic expansion member (16) for self-expanding said open distal end (15) to an expanded configuration; and a pursing member (20) which is arranged to purse said flexible tubing (11). A method for sealing a puncture in a body vessel, comprises introducing a distal end of the sealing member through the puncture; allowing the elastic expansion member to self-expand to an expanded configuration in the vessel, detecting a blood flow through the internal passageway as an indication of the distal end of the sealing member being positioned inside the body vessel, and thereafter sealing the internal passageway by pursing the flexible tubing outside the body vessel.

Disclosed are methods and compositions for closing and sealing a wound, laceration, incision, or other percutaneous opening using an adhesive. In one preferred embodiment, the sides of the percutaneous opening are brought together in apposition and the adhesive is applied to the opening and the skin immediately adjacent thereto. Adhesives used in the methods of the present invention exhibit sufficient viscosity to substantially prevent flow of the adhesive into the percutaneous opening. In a preferred embodiment, the adhesive is a sealing medium comprising an adhesive component and a microparticulate component. Preferred sealing media comprise cyanoacrylates combined with filmed silica. In such embodiments, said viscosity enhancing component is present in an amount sufficient to substantially prevent the medium from flowing into the opening at normal skin temperature.

A wound closure system and methods utilizes a patient's own blood. The system preferably includes a chamber with a needle and a plunger for transferring and storing blood prior to surgery and the use of reversible anticoagulants. The chamber is pre-loaded with a reversible anticoagulant to prevent the blood from clotting during storage. When the surgery is complete, a counteracting agent is mixed with the withdrawn blood to counteract the reversible anticoagulant and allow the blood to clot. The clottable blood is then reintroduced into the patient to close a wound.

A surgical system includes an endoscope, a pair of end effectors provided on the outside of the distal end of a endoscope (nullon-scopenull), in which one end effector is rotatable or otherwise movable relative to the second end effector and in which such movement is effected via controls elements which do not extend through a lumen of the endoscope, and another instrument extending through the lumen of the endoscope (nullthrough-scopenull), and which preferably also has a pair of end effectors. The on-scope device may be clamping jaws, needle passers, clip appliers, cautery devices, staplers, and releasable hemoclips, among other end devices. The through-scope end effectors may be graspers and clip appliers, among other end effectors.

A biocompatible and biodegradable barrier material is applied to a tissue region, e.g., to seal a vascular puncture site. The barrier material comprises a compound, which is chemically cross-linked without use of an enzyme to form a non-liquid mechanical matrix. The compound preferably includes a protein comprising recombinant or natural serum albumin, which is mixed with a polymer that comprises poly(ethylene) glycol (PEG), and, most preferably, a multi-armed PEG polymer.

An apparatus for sealing a passage through tissue includes a tubular member, a plug, and a locator member. The plug is disposed on a distal end of the tubular member and includes an external thread. The locator member is inserted into a lumen of the tubular member until a distal portion is disposed beyond the plug. The distal portion includes a helically wound wire and a tether coupled to the wire that is movable for causing the wire to buckle. To seal a passage communicating with a blood vessel, the distal portion of the locator member is inserted into the passage, and the plug is threaded into the passage until the distal portion extends into the vessel. The tether is pulled to buckle the wire, and the plug is unthreaded until the buckled wire contacts a wall of the vessel, whereupon the plug is released to seal the passage.

An absorbable sponge containing a contrasting agent (e.g, radiopaque agent) that can be introduced to a biopsy tract or other puncture wound site is provided. The contrasting agent permits identification of the site by fluoroscopy or other imaging techniques.
