Abstract: A system and method of implanting pacing lead in a patient's heart. The system may include a catheter configured to by inserted through the coronary sinus ostium such that the distal end region of the catheter is positioned past the anterolateral vein and proximate at least one septal perforating vein. The catheter is configured to inject contrast proximate the septal perforating vein to identify an implant region for a pacing lead. Further, a controller is configured to deliver pacing therapy to the implant region.

One or more local metrics of electrical heterogeneity information may be generated based on surrogate cardiac electrical measured using a plurality of local external electrodes, which may be used to determine whether the patient's cardiac conduction system is engaged.

Abstract: An implantable medical device includes a plurality of electrodes to detect electrical activity, a motion detector to detect mechanical activity, and a controller to determine at least one electromechanical interval based on at least one of electrical activity and mechanical activity. The activity detected may be in response to delivering a pacing pulse according to an atrioventricular (AV) pacing interval using the second electrode. The electromechanical interval may be used to adjust the AV pacing interval. The electromechanical interval may be used to determine whether cardiac therapy is acceptable or whether atrial or ventricular remodeling is successful.

Abstract: In some examples, controlling delivery of CRT includes controlling an implantable medical device to deliver ventricular pacing according to a sequence of different values of a CRT parameter, and acquiring first and second electrograms from respective first and second electrode vectors. For each value of the CRT parameter, a value of a metric of comparison of a first activation interval between occurrences of a first fiducial of a cardiac cycle and a second fiducial of the cardiac cycle detected in the first electrogram to a second activation interval between occurrences of the first fiducial and the second fiducial detected in the second electrogram may be determined. A target value of the metric of comparison may be identified and an updated value of the CRT parameter determined based on the target value. The system then may control the IMD to deliver ventricular pacing at the updated value of the CRT parameter.

Abstract: Systems for cardiac therapy using pseudo-electric vectors (PEVs) for characterizing and representing the electrical forces generated by a patient's heart in a three-dimensional (3D) manner are disclosed. PEVs may be used to predict whether a patient will respond to pacing therapy prior to implant, during implant, or in the follow-up after implant. Various cardiac therapy systems and devices, such as an electrocardiogram (ECG) belt or vest, which may include a plurality of external electrodes, may be used to obtain electrical activity information to generate the PEVs. One or more spatio-temporal PEVs may be determined using one or more sensors at one or more points in time. Spatial representation data may be determined based on the PEVs.

Abstract: Capture management in a left ventricular leadless pacing device that includes determining an intrinsic P-wave of a sensed cardiac signal, sensing an electromechanical signal from an electromechanical sensor of the pacing device, and determining an intrinsic electromechanical atrioventricular interval of the sensed electromechanical signal in response to the sensed P-wave. Ventricular pacing is delivered via the one or more electrodes of the pacing device, and a ventricular pacing (V-pace) event is determined in response to the delivered ventricular pacing, and a V-pace to electromechanical response interval is determined in response to the V-pace event. A determination as to capture is detected is made in response to the intrinsic electromechanical atrioventricular interval and the V-pace to electromechanical response interval, and a pacing parameter is determined in response to determining whether capture is detected.

Abstract: The disclosure describes systems and techniques for detection of pump thrombosis in mechanical circulatory support (MCS) devices. An example pump thrombosis detection system includes a transducer and processing circuitry. The transducer may be configured to generate a signal representative of a mechanical wave from a mechanical circulatory support device. The processing circuitry is communicatively coupled to the transducer. The processing circuitry may be configured to determine an indication of pump thrombosis based on the signal and, based on the indication of pump thrombosis, control the pump thrombosis detection system to at least one of generate an alert or initiate an intervention.
