摘要: The present invention relates to an implantable medical device for stimulating a human or animal heart, comprising a processor, a memory unit, a first stimulation unit configured to stimulate an atrium of a human or animal heart, a second stimulation unit configured to stimulate a ventricle of the same heart, a first detection unit configured to detect an electric signal in the atrium, and a second detection unit configured to detect an electric signal in the ventricle. In operation, the implantable medical device performs the following steps: a)detecting, with the first detection unit, an electric signal (A) of a heart to be stimulated, the electric signal (A) comprising at least two consecutive amplitudes (P, P') correlating with a contraction of the heart to be stimulated; b) determining i) whether a first amplitude (P, P') of the two consecutive amplitudes (P, P') and a second amplitude (P, P') of the two consecutive amplitudes (P, P') have a maximum lying within a predeterminable intensity range (5, 6), and/or ii) whether a maximum of the first amplitude (P, P') and a maximum of the second amplitude (P, P') deviate from each other in terms of intensity by not more than 20 %; and c) preventing an atrial stimulation with the first stimulation unit if at least one ofconditions i) and ii) of step b), if tested, is not fulfilled, otherwise allowing an atrial stimulation with the first stimulation unit.

摘要: Systems and methods for cardiac pacing during a procedure are disclosed and may include an external pulse generator (EPG) for connecting to a lead. A remote-control module (RCM) wirelessly connected to the EPG may include user inputs to control the EPG. A central processing unit (CPU) with a memory unit for storing code and a processor for executing the code may be included where the CPU is connected to the EPG and RCM. The code may control the EPG in response to user input from the RCM. The CPU may be disposed in the EPG or the RCM, or an interface module (IM) configured to communicate between an otherwise conventional EPG and the RCM. The executable code may perform a continuity test (CT) routine, a capture check (CC) routine, rapid pacing (RP) routine, and/or a back-up pacing (BP) routine, in response to user input from the RCM.

A medical device is configured to sense at least one cardiac electrical signal and generate pacing pulses according to a first pacing therapy by generating pacing pulses for delivery to a first combination of ventricular pacing sites including at least one ventricular conduction system pacing site. The medical device may be configured to determine a ventricular conduction condition based on a QRS signal feature of the sensed cardiac signal during the first pacing therapy. The medical device may change to a second pacing therapy different than the first pacing therapy based on the determined ventricular conduction condition.

摘要: Disclosed are techniques to generate ideal or near ideal profiles for regulation of the fluid flow in a drive system of a pump for an externally mechanically supported heart, pressure in or near the pump, or measured strain/ strain rates of the supported heart, based on an estimate/measurement of the heart's size represented by a volume occupied by the heart's muscles as well as blood contained within chambers of the heart. A part of the techniques for regulation may focus on achieving mechanical synchrony with the intrinsic cyclic pump function of a partially functional heart. The techniques do not fundamentally rely on hemodynamic measurements to function. However, when hemodynamic measures are available, those measures can be fed to control algorithms to increase the efficacy of regulation to restore the heart's pump function.
