A method and system provide and use a patellar implant. The patellar implant includes a superior portion, an inferior portion opposite to the superior portion, and an anterior portion. The superior portion being configured to reside below a patellar tendon and to elevate and/or tilt the patellar tendon. The inferior portion is configured to be seated in proximity to a tibia. The anterior portion is between the superior portion and the inferior portion. The anterior portion is placed in proximity to a patella. In one aspect, the method includes inserting the implant beneath the patellar tendon and between the patella and a position at which the patellar tendon is affixed to the tibia. In this aspect, the method also includes affixing the implant.

The embodiments provide devices and methods that both strengthen the bone and stimulate the bone. Bone fractures or non-unions are stabilized, integrated or healed, which results in reduction of a bone defect, such as a bone marrow lesion or edema. In addition, the distribution of forces in a joint are restored or altered to relieve pain. In general, a joint is evaluated by taking an image of the joint and one or more subchondral defects are detected. At least one of the subchondral defects may be diagnosed as the source of pain and an extent of treatment for the subchondral defect is determined. The disclosed devices and techniques are particularly suited for treating chronic defects or injuries, where the patient's natural healing response has not resolved the defect. The present disclosure also provides several exemplary treatment modalities for the different extents of treatment needed.

A tissue scaffold fabricated from bioinert fiber forms a rigid three-dimensional porous matrix having a bioinert composition. Porosity in the form of interconnected pore space is provided by the space between the bioinert fiber in the porous matrix. Strength of the porous matrix is provided by bioinert fiber fused and bonded into the rigid three-dimensional matrix having a specific pore size and pore size distribution. The tissue scaffold supports tissue in¬ growth to provide osteoconductivity as a tissue scaffold, used for the repair of damaged and/ or diseased bone tissue.

A prosthetic tibial insert comprises a tibial tray insert including a stabilizing post, where the centerline of the stabilizing post is offset from at least one of an anterior-posterior centerline and a medial-lateral centerline of the tibial tray insert. A method of imparting axial rotation comprises shifting the axis of axial rotation between a tibial prosthesis and a femoral prosthesis by providing at least one of offsetting a stabilizing post associated with a tibial insert, increasing the radii of a medial receiver of the tibial insert, and increasing conformity between a medial receiver of the tibial insert and a medial condyle.

A biomedical implant is disclosed with osteogenic factors and a solid impermeable membrane occluding a portion of its surface for the generation of new bone growth at the target site of the implant. The implant is porous, bioresorbable, and forms a three dimensional architectural scaffold for the formation of new bone tissue. The implant is formed with a polymer or collagen, bone morphogenetic protein and ceramic particles.

It is an object of the present invention to provide a figure-forming composition which enables the initiation of hardening even with a small amount of water and can attain a prescribed hardness, and which is suitable particularly for the production of three-dimensional living body models for surgical training by a rapid prototype process; and provide a three dimensional figure and a three-dimensional structure which are formed by using the composition. The figure-forming composition, for example, comprises a calcium-based material such as hemihydrate gypsum and polyvinyl alcohol resin, wherein the content of the polyvinyl alcohol resin is 2 to 20 weight parts when the total weight of the calcium-based material and the polyvinyl alcohol resin is 100 weight parts.

A combination dense/porous structure includes a porous element having an outer surface defining a shape having a bulk volume, the element includes a continuous framework having struts defining a plurality of interconnecting interstices throughout the bulk volume. The porous element has interconnecting interstices extending throughout the volume and opening through the surface. The combination dens/porous structure also includes a dense element formed from a material and having a sintered density of at least 95%, contacting at least a portion of the porous element; and an interconnection zone formed by the inter-penetration of the material forming the dense element into the porous element. The combination dense/porous structures are particularly useful as bone substitute materials and extended release delivery systems, preferable sustained release, for physiologically active agents. The interconnection zone can also be formed by a bonding phase that may or may not be the same material as the dense and/or porous element. A process for producing a combination dense/porous structure includes: providing a porous element; providing a dispersion comprising a ceramic or metal powder and a binder; contacting the dispersion with the porous element whereby the slip at least partially penetrates into at least a portion of the porous element to form an interconnection zone and a dense element formed from the dispersion and adjacent to the interconnection zone to form a dense/porous structure; treating the dense/porous structure to form a green dens/porous structure; and curing, drying, and/or heating the green dense/porous structure to form the combination dense/porous structure.
