    摘要: A surgical access apparatus adaptable to permit the sealed insertion of either the surgeon's hand and/or surgical instruments during laparoscopic and endoscopic surgical procedures includes an access housing defining a longitudinal axis and having a first internal passageway configured and dimensioned to permit passage of at least one of a hand and an arm of a surgeon, and a base mountable to the access housing. The base may include a liner member positionable within an incision of a patient to at least partially line the incision. A displacement member may be operatively connected to the access housing and to the liner member. The displacement member is adapted for movement to cause corresponding displacement of the liner member relative to the access housing whereby the liner member engages tissue forming the incision to at least partially retract the incision.

    摘要: A surgical portal apparatus includes a portal housing, a portal sleeve, and a seal. The portal housing defines a central housing axis and a central housing channel. The portal sleeve extends from the portal housing and is dimensioned to pass through tissue to provide access to underlying tissue via a longitudinal opening. The central housing channel of the portal housing and the longitudinal opening of the portal sleeve define a passageway for reception and passage of a surgical object. The seal has inner surfaces defining a seal passage for establishing a general sealed relation about the surgical object. The seal passage is radially offset with respect to the central housing axis. The seal is adapted to rotate about the central housing axis to vary positioning of the seal passage to substantially maintain the substantial sealed relation upon manipulation of the surgical object within the portal housing.

The first and second lips each include a proximal lip surface having an outer surface portion and an inner surface portion interconnected by a curved surface. A support rib depends from the proximal lip surface of each of the first and second lips to define a gap therebetween to be in spaced relation when the valve is in the closed position and being positioned to contact the surgical instrument during insertion through the first and second lips.

    摘要: A surgical access device includes a surgical sleeve defining a central longitudinal axis and having a longitudinal passageway to permit passage of a surgical object, and a diameter reduction assembly operably couplable to the surgical sleeve. The diameter reduction assembly includes a structure foundation and at least three stand-off elements mounted to the structure foundation. Each stand-off element is positioned to engage the surgical object and is independently movable relative to the remaining stand-off elements. The at least three stand-off elements are normally biased to a first position which urges the surgical object into general alignment with the central longitudinal axis, and are adapted for distal pivotal movement from the first position to a second position to facilitate passage of the surgical object, and for proximal pivotal movement from the first position to a third position to facilitate withdrawal of the surgical object.

The tissue engaging member includes a membrane having proximal and distal ends with opposing openings, and a passage therethrough for receiving the access member. A length of the tissue engaging member may be selectively adjustable by a user so as to provide a retraction force. The sleeve extends proximally from a portion of the membrane. The sleeve has a proximal end and a distal end with opposing openings, the distal end of the sleeve defined by a portion of the membrane. A fastening member couples the access member and the sleeve.

    摘要: A surgical access assembly for use with a surgical access device includes a housing defining a central longitudinal axis, an elongate access member extending from the housing whereby the housing and the access member having a longitudinal passage dimensioned to permit passage of a surgical instrument, an instrument seal disposed within the housing and being adapted to establish a substantial fluid tight seal about the instrument and a zero closure valve mounted relative to the housing and being adapted to assume an open condition upon passage of the surgical instrument and a substantially closed position in the absence of the instrument. The zero closure valve includes a valve periphery and first and second lips depending from the valve periphery. The first and second lips have lip edge surfaces in juxtaposed relation to define a slit when the valve is in the closed position thereof. At least one of the lips has a general arcuate internal lip surface leading to the lip edge surface to define a sloping configuration dimensioned to facilitate passage of the surgical instrument through the valve. Each of the first and second lips may define a general arcuate lip surface. In one embodiment, the internal arcuate lip surface of each of the first and second lips includes an outer surface portion arranged at a first angle relative to the longitudinal axis of the seal housing and an inner surface portion arranged at a second angle relative to longitudinal axis and being different from the first angle.
