A sensor placement and retention mechanism for use with fetal sensor sites beyond the user's reach are disclosed. The preferred embodiment of this invention is a fetal pulse oximetry sensor having an active face through which a light source and a light detector operate. The sensor includes a handle that facilitates placement of the active face at a sensor site in a preferred region beyond the transcervical region and beyond the reach of the user. A pair of electrodes--one disposed against the fetus' skin and one exposed to the amniotic fluid--are used to confirm that the sensor is firmly in place on the fetus. In an optional sensor retention feature, a self-inflating bladder presses the active face of the sensor against the fetus' skin to optically couple the sensor with the tissue at the sensor site.

A first light emitting element directs light through a first region of tissue, a second light emitting element directs light through a second region of tissue that is spatially distinct from the first region of tissue, and a photodetector detects light traveling through the first and second regions. First and second oxygen saturation values are calculated from the amount of light detected from the first and second regions of tissue, respectively. A difference calculating unit calculates a difference value which represents how much the first oxygen saturation value differs from the second oxygen saturation value, and a comparator compares the difference value to a threshold value. A threshold indicating signal is provided when the difference value has a selected mathematical relationship (e.g., less than or equal) to the threshold value.

A sensor has codes useful for a monitor which can be authenticated as accurate. The sensor produces a signal corresponding to a measured physiological characteristic and provides codes which can be assured of being accurate and authentic when used by a monitor. A memory associated with the sensor stores both data relating to the sensor and a digital signature. The digital signature authenticates the quality of the code by ensuring it was generated by an entity having predetermined quality controls, and ensure the code is accurate.

Forehead oximetry sensor devices and methods for determining physiological parameters using forehead oximetry sensors. One method includes placing an oximetry sensor on the forehead of a patient, such that the sensor is placed on the lower forehead region, above the eyebrow with the sensor optics placed lateral of the iris and proximal the temple; and operating the pulse oximeter to obtain the physiological parameter. In one aspect, the method also includes providing and placing a headband over the oximetry sensor, or alternately, the sensor is a headband-integrated sensor. The headband has an elastic segment sized to fit around the patient's head. The headband also includes a non-elastic segment that is smaller than and attached with the elastic segment. The non-elastic segment is sized to span a portion of the elastic segment when the elastic segment is stretched. In addition, the non-elastic segment is larger than the portion of the elastic segment it spans when the elastic segment is not stretched. When the headband or the headband-integrated sensor is sufficiently tight, it delivers a pressure in the range higher than the venous pressure and lower than the capillary pressure to the forehead of the patient.

The method continuously calculates a baseline value, and establishes dynamic thresholds that are related to and continuously track the baseline value. The method determines the amount of time the measured value is past the dynamic threshold, and the amount by which the threshold is passed. Alarms are triggered based upon a combination of the amount of time and the amount by which the threshold is passed. Preferably, the combination is an integral or some function of an integral.
