摘要:
The invention provides a method for replacing a first introducer, whose distal end is inside a vessel and whose proximal end is outside the skin of a patient, with a second introducer. The method comprises the steps of inserting a guide rod into the first introducer, removing the first introducer, passing the second introducer over the guide rod, and removing the guide rod. The guide rod to be used according to the method has preferably a flexible distal portion with a blunt distal end and a stiff proximal portion with a tapered proximal end.

The invention relates to a catheter arrangement for invasive medical treatment for introducing and controlling a guide wire in a vessel branch or vessel entry by bending the guide wire. The catheter arrangement comprises two catheters, each having a distal end and a proximal end and a lumen, wherein the two catheters are embodied as an inner catheter 3 and an outer catheter 2 extending coaxially within one another and arranged so as to be moveable relative to one another in an axial direction, wherein the inner catheter 3 has a pre-bent distal tip 3a of a bendable material and the outer catheter 2 has a continuous lumen 22 extending from the proximal end 20 to the distal end 21 for receiving the inner catheter 3, wherein both catheters 2, 3, at a spacing from the distal end 21, have lateral openings 23, 33 for passing the guide wire F therethrough.

A system and method for navigating coronary vasculature involves use of a guide catheter system which includes a guide catheter, a navigator catheter longitudinally displaceable within the guide catheter, and a deflection arrangement provided at a distal end of the navigator catheter. The guide catheter is advanced to at least a patient's coronary sinus ostium, and the navigator catheter is extended from the guide catheter to a location proximate or within an angled vein distal to the coronary sinus ostium. Using the deflection arrangement, a guide wire passing through the navigation catheter is directed into the angled vein. A lead having an open lumen is advanced over the guide wire to direct the lead to an implant site within the angled vein.

The invention is a method for manufacturing a steerable catheter having a distal end, proximal end, an outer jacket, a pull wire and a central lumen. The central lumen is maintained in a circular shape without bulges diminishing the useful inter-diameter by using an outer jacket with an elliptical shape and uneven thickness to encase a pull wire. The pull wire friction is also reduced by using one wire of larger diameter to create the lumen for the pull wire of smaller diameter, thus reducing the friction on the pull wire and reducing the locking of the catheter body around the pull wire at bends preventing movement of the pull wire. The distal end of the catheter also has a smooth tip due to heat treating of steel wire braid reinforcement that does not require the addition of bulge forming additional material to control tip shape.

Guide catheters which can be used in percutaneous myocardial revascularization (PMR) to deliver therapeutic catheters to difficult to reach heart chamber wall regions. Some guide catheters include distal regions which can be bent under control from the proximal region of the catheter. One steerable guide catheter has a flexible distal region, a more proximal, less flexible intermediate region, a first lumen for receiving a therapeutic catheter, and an elongate manipulation member slidably disposed in a second, blind lumen. The elongate manipulation member can be secured off-center near the distal end of the flexible distal region. The distal region can be bent by retracting the manipulation member and straightened by pushing the manipulation member. Controllably bendable guide catheters according to the present invention can be nested inside other, similar guide catheters. The invention also includes means for resisting free rotation of guide catheters relative to other adjacent catheters or tubes.

The present invention relates to a catheter incorporating an articulating distal end, including a catheter body with a central lumen and an outer wall in which are one or more wires, and a handle including mechanisms for applying tension to the wires. Applying tension to a wire will cause a bending portion of the catheter to bend into an arc. When more than one wire or tensioning member and more than one mechanism for applying tension are included in the catheter, the wires are offset from each other so that multiple bends on multiple planes may be formed in multiple bending portions in the body of the catheter. The present invention further relates to a catheter including one wire, and a handle including a mechanism for applying tension to the wire, the wire and the catheter body configured so that applying tension to a wire will cause more than one bending portion of the catheter to bend into an arc, and the bending portions are made of different durometer material. The present invention further relates to a method of using a catheter to access a heart chamber of a patient and locate the distal end of the catheter proximate a shunt located in a wall of the heart.

A micro catheter and guidewire system for use in tortuous regions of a vasculature includes a catheter body having a distal end, a guidewire lumen, a proximal region with an outer diameter, and a distal region with an outer diameter. The ratio of the proximal region outer diameter to the distal region outer diameter is at least 1.625:1 to improve pushability of the catheter body. The guidewire lumen defines a stopper and the guidewire has a depth stop for engaging the stopper of the guidewire lumen. The depth stop and the stopper cooperate to enable the guidewire to cooperate with the catheter to improve catheter pushability. The depth stop and the stopper also prevent over extension of the guidewire.

A transformable catheter includes a sheath having a length which is a substantial fraction of the entire length of the transformable catheter and a bore therethrough. A preformed inner catheter having a complex curve formed into the distal end thereof is disposed in the sheath bore. By axially moving the inner catheter with respect to the sheath, various tip shapes may be achieved. At least one wire runs from the proximal end of the sheath to the vicinity of the distal end of the sheath to selectively deflect the distal tip of the sheath as desired by the user to assist in reformation and transformation of the tip of the catheter. By suitable manipulation of the wire and of the inner catheter with respect to the sheath, the shape of the exposed portion of the distal end of the inner catheter may be transformed to any of a variety of shapes.
