    摘要: An electrosurgical instrument includes a housing having a shaft attached thereto which connects a pair of first and second jaw members in an opposing manner relative to one another. The jaw members are movable relative to one another from a first open position wherein the jaw members are disposed in spaced relation relative to one another to a second closed position wherein the jaw members cooperate to grasp tissue therebetween. The instrument also includes a handle which imparts movement of the first and second jaw members from the first and second positions and a rotating assembly for rotating the jaw members about a longitudinal axis defined through the shaft. A pair of electrical leads connect the jaw members to a source of electrical energy such that the jaw members are capable of conducting energy through tissue held therebetween. The leads include a slack-like loop disposed in the rotating assembly which permits rotation of the jaw members about the longitudinal axis without unduly straining the electrical leads.

The proximal portion has an articulating arm for repositionably mounting to a base and the distal portion supporting a stabilizing member. An attachment mounts on the stabilizing member, the attachment being configured and dimensioned such that upon contact with the heart the engaged heart tissue bulges outward. A method of surgery using the attachment includes providing a frame structure defining proximal and distal portions, the proximal portion being repositionably mountable to a base, supporting an attachment on the stabilizing member and engaging heart tissue with the attachment such that the tissue bulges outward to better present the tissue for surgery.

    摘要: A surgical biopsy apparatus is provided which includes a housing; a first elongated tubular member removably mounted in the housing and defining a fluid passageway therein, the first elongated tubular member including: a tapered closed distal end portion adapted to penetrate tissue; a laterally disposed tissue receiving opening formed near the tapered distal end which includes a tissue support surface defining a plurality of holes in fluid communication with the fluid passageway; and a second elongated tubular member rotatably and reciprocatingly disposed coaxially about the first elongated tubular member, the second elongated tubular member having a cutting edge formed at an open distal end thereof and a lateral tissue discharge port formed proximally of the cutting edge.A method of performing a surgical biopsy is also provided which includes inserting a biopsy apparatus into the tissue of a patient, the biopsy apparatus including an inner tubular member having a tapered penetrating end formed on the distal end and an outer tubular member held longitudinally fixed relative to the inner tubular member during the insertion step; retracting the outer tubular member relative to the inner tubular member to expose a laterally disposed tissue receiving area formed on the inner tubular member; applying suction to a series of openings formed along an inner surface of the tissue receiving area to pull tissue into the tissue receiving area; severing tissue disposed within the tissue receiving area by advancing the outer tubular member over the inner tubular member such that a cutting surface formed on the distal end of the outer tubular member rotates as it passes over the tissue receiving area; and removing the severed tissue sample from the tissue sampling site by retracting the inner tubular member from the outer tubular member until the tissue receiving area is aligned with a lateral opening formed in the outer tubular member wherein a tissue stripping plate urges the tissue sample out of the tissue receiving area.

    摘要: A loading unit is disclosed for supplying multiple surgical needles or surgical incision members and associated lengths of suture material for positioning the needle or surgical incision member within the jaws of a surgical suturing apparatus. The loading unit generally includes a body portion having apparatus receiving structure for receipt of the surgical suturing apparatus and a carousel rotatably affixed to the body portion and containing a plurality of supply stations thereon. The supply stations generally include a support member for supporting a central portion of a surgical needle and a storage member for temporarily securing a length of suture material associated with the surgical needle. The apparatus receiving structure includes structure for aligning and maintaining the surgical suturing apparatus in position on the loading unit until such time as a surgical incision member has been inserted into jaws associated with the surgical suturing apparatus. Preferably, the apparatus receiving structure includes structure for gasping an elongate portion of the surgical suturing apparatus and structure for supporting jaw members associated with the surgical suturing apparatus. Additionally, in a preferred embodiment, the suture storage members consist of a suture reel which is positioned in a plane offset from the needle support members.

The distal slide member is positioned adjacent the first lateral side of the thrust bar and is adjacent the distal portion of the thrust bar. The proximal portion of the distal slide member is adapted to slide relative to the thrust bar when thrust bar moves in a curvilinear direction.

The jaw members are each longitudinally curved with respect to the longitudinal axis. The dynamic clamping member is disposed in mechanical cooperation with a distal portion of the drive assembly and includes an upper beam, a lower beam, and a vertical beam having a cutting edge on a distal portion thereof. At least a portion of the dynamic clamping member is longitudinally curved with respect to the longitudinal axis. The tissue stop is disposed adjacent a distal portion of the first jaw member and configured to impede tissue from distally escaping the jaw members.
