    摘要: An obturator assembly for penetrating tissue and being at least partially positionable within a cannula assembly includes an obturator housing, an obturator shaft connected to the obturator housing and defining a longitudinal axis, a bladeless penetrating member adjacent the distal end of the obturator shaft and an obturator shield mounted about the obturator shaft and having a distal shield nose. The obturator shield is adapted for longitudinal movement between an extended position where the bladeless penetration member is substantially enclosed within the shield nose and a retracted position where the bladeless penetrating member is at least partially exposed from the shield nose. A manually manipulative member is mounted to the obturator housing and is adapted for rotational movement about the longitudinal axis and relative to the obturator housing between an initial position corresponding to a first mode of operation where the obturator shield is secured in the extended position and the shield nose is used to penetrate tissue, and a release position corresponding to a second mode of operation operatively releasing the obturator shield to permit the obturator shield to move to the retracted position thereof to expose the bladeless penetrating member for further dissecting the tissue.

    摘要: Systems, kits, and methods for establishing percutaneous access are described. A system typically includes a deformable variable diameter cannula sleeve (DVDCS) and a guidewire. The DVDCS will generally have two states, an initial, un-activated condition and a second, activated condition, brought about and controlled under the discretion of the operator. The methods comprise creating an initial access point and tissue tract with a needle, positioning a guidewire through the initial tissue tract, passing the deformable variable diameter cannula sleeve over the guidewire through the tissue tract to a target blood vessel or cavity, and activating the DVDCS to effect radial deformation of the surrounding tissue. Use of the DVDCS reduces the risk of injuring tissue surrounding the tissue tract by lessening the axial forces imparted to the tissue. Kits comprise at least one deformable variable diameter cannula sleeve together with instructions for use.

    摘要: A trocar system includes a cannula having a cannula housing and a cannula sleeve extending from the cannula housing, and an obturator at least partially positionable within the cannula. The obturator includes an obturator housing adapted for mating with the cannula housing, an obturator shaft connected to the obturator housing and receivable within the cannula sleeve, an obturator blade connected to the obturator shaft, a protective member including a protective guard coaxially mounted about the obturator blade and a latch member associated with the obturator housing and operatively connected to the protective guard. The obturator shaft and the obturator blade are adapted for relative rotational movement. The protective guard is adapted for longitudinal movement between a first position substantially enclosing the obturator blade and a second position to at least partially expose the obturator blade. The latch member is moveable from an initial position securing the protective guard in the first position to an actuated position operatively releasing the protective guard to permit the protective guard to move to the second position thereof. The latch member is preferably adapted to move to the actuated position upon mating of the obturator housing and the cannula housing.

    摘要: A surgical instrument for use with a surgical portal apparatus includes an elongate shaft having proximal and distal ends and defining a longitudinal axis and a dilating member disposed at the distal end of the elongate shaft. The dilating member has a first longitudinal arc segment and a second longitudinal arc segment in diametrical opposed relation to the first longitudinal arc segment. The first and second longitudinal arc segments define respective first and second radii of curvature transverse to the longitudinal axis. The first and second radii of curvature decrease from proximal to distal to define a generally tapered configuration of the dilating member. The first radius of curvature is greater than the second radius of curvature at predefined longitudinal positions of the dilating member such that the first longitudinal arc segment extends distally beyond the second longitudinal segment to define a penetrating tip. The penetrating tip has a reduced profile and is dimensioned to facilitate initial insertion of the dilating member into tissue.
