This patent document discusses, among other things, apparatuses and methods including an expandable stimulation electrode with an integrated pressure sensor. In various examples, the apparatus further comprises a pulse generator, a controller, a posture sensor, or a physiological parameter sensor. When expanded, the electrode is adapted to abut a wall of a pulmonary artery, thereby providing an arterial anchor for the integrated pressure sensor. In addition, the expandable electrode provides a means to deliver baroreflex stimulation signals, generated by the pulse generator, to one or more baroreceptors in the arterial wall. Based on pressure sensor-provided signals indicative of an arterial blood pressure, the controller provides stimulation instructions to the pulse generator. The posture sensor may be used to normalize the pressure data or limit such data collection to a single posture orientation. In one example, the physiological parameter sensor includes a temperature sensor.

Systems and methods obtain information that allows detection of whether a patient suffers from congestion by monitoring a response of the patient to a recumbent position of the patient's body. The patient may be monitored to determine a respiration pattern for a non-recumbent position such as standing and a respiration pattern for a recumbent position such as lying down. The two patterns may be compared, either by a processing device or a physician, to determine a difference in the two respiration patterns. Furthermore, the congestion may be inferred from detecting an amount of time that the patient spends in a recumbent position or from detecting the recumbent angle that the patient obtains, either of which is presumed to indicate whether a recumbent position presents discomfort to the patient due to the presence of congestion.

A system, method, or device classifies an arrhythmia according to the temporal order in which a depolarization wave associated with a particular heart contraction is received at a plurality of electrodes. One or more antiarrhythmia therapies is mapped to each arrhythmia classification. When a particularly classified arrhythmia is detected, the correspondingly mapped therapy list is selected and an appropriate antiarrhythmia therapy delivered. In one example, the particular therapy delivered in response to an arrhythmia depends at least in part on its historical success in treating arrhythmias of that classification.

Adventitious lung sounds indicative of lung congestion are detected using an implantable sensor. The sensor is adapted to be positioned adjacent to a pulmonary system and to output signals indicative of lung sounds in response to pulmonary system activity. A controller receives the signals and processes the signals to detect the presence of adventitious lung sounds. A respiratory cycle sensor operating in conjunction with the lung-sound sensor enables classification of an adventitious lung sound according to its time occurrence within the respiratory cycle. Posture sensing in conjunction with lung-sound sensing provides valuable additional information as to the severity of the lung congestion.

A method of optimizing cardiac resynchronization therapy delay over a patient's full range of activity for use in operating an implantable cardiac pacing device and such a device are disclosed. The method includes measuring selected conduction time between selected sites in the heart for a plurality of beats and logging the values on a periodic repeating programmable basis to produce cumulative data and constructing a current template of conduction time in relation to one or more other sensed parameters of interest over a desired range of patient activity levels. The current template is used to derive suggested optimum pacing timing.

One aspect relates to a method to provide trended measurement of cardiac resynchronization therapy (CRT). In various embodiments of the method, data related to a status of a prescribed cardiac resynchronization therapy (CRT) in a cardiac rhythm management (CRM) device is recorded, and the data is processed into trended data useful for assessing the status of the prescribed CRT. The trended data is presented for use to assess the status of the prescribed CRT and assist with determining programming adjustments for improving the prescribed CRT. The system includes an implantable device communicating with a transdermal drug delivery device. The transdermal drug delivery device delivers a pharmaceutical substance in response to a detection of the condition by the implantable device. In one example, the implantable device detects the condition by monitoring a hemodynamic performance. In another example, the implantable device detects the condition by sensing a signal indicative of decompensation.
