    摘要: The invention is directed to a stent delivery method and system which generally includes an elongated delivery sheath and a catheter disposed within an outer lumen of the sheath having an expandable member on its distal extremity. An expandable stent is mounted on the expandable member of the catheter. The distal portion of the sheath tapers down and is tucked within an elastic cone during transport of the stent to a stenotic region. A manipulating device is provided on the proximal end of the delivery system to effect relative axial movement between the sheath and the catheter so as to expose the stent mounted on the expandable member on the catheter within a body lumen such as a coronary artery and allow the expansion of the stent by the expansion of the expandable member. The elastic cone thereby disengages from the sheath and collapses about the distal end of the catheter. The delivery sheath has a first port in its distal end and a second port in the sheath wall proximally disposed from the distal end of the sheath. The catheter likewise has a first port in its distal end and a second port proximally disposed from the distal end of the catheter. An inner lumen extends within the distal portion of the catheter between the first and second ports and slidably receives a guiding member such as a guidewire. This system allows the stent to be delivered over a guidewire previously advanced to the desired location within a body lumen.

    摘要: An intravascular stent including a cylindrical sheet having overlapping edges that interlock. The edges have a series of protrusions and apertures that interlock and ratchet as the stent expands to an open position to support a section of arterial wall. The stent may be expanded by a balloon catheter or it may be self-expanding. The stent is biocompatible, may be bio-erodible, and capable of localized drug delivery. A plurality of retaining members to keep the stent open are disclosed. In one embodiment a buckle fastening member is used, while in another embodiment a helical seam containing projections is employed. The stent may be wound in such a manner that during expansion of the stent one side of the sheet desires to return to its original shape, creating a bias. In addition, a variety of reticulated structure stents are disclosed, with novel geometric patterns that aid in increased flexibility while preserving radial strength and also allow blood-tissue interaction and side branch access. The intravascular stent may be made of a sheet of material strengthened and stiffened by pyrolytic carbon or by structural reinforcement as in composite laminates.

    摘要: Apparatus for sealing a puncture communicating with a blood vessel includes an introducer sheath, an occlusion member with a balloon thereon, a retraction assembly, and a syringe assembly including sealing material therein. During use, the introducer sheath is introduced into the puncture. With the balloon in a collapsed state, the occlusion member is introduced through the introducer sheath until the balloon is disposed within the vessel. The introducer sheath is coupled to the retraction assembly, the balloon is expanded, and the occlusion member is partially withdrawn to seal the puncture. A sealing compound is delivered from the syringe assembly through the introducer sheath into the puncture, the syringe assembly triggering the retraction assembly, whereupon the introducer sheath is automatically withdrawn at least partially from the puncture, delivering the sealing compound along the puncture.

    摘要: Apparatus for sealing a puncture through tissue to a blood vessel includes a cartridge including a proximal end, a distal end sized for insertion into a puncture, and a lumen extending therebetween. A bioabsorbable plug is disposed within the lumen adjacent the distal end, and an anchoring element is disposed within the lumen proximal to the plug. A pusher member is disposed within the lumen for deploying the plug and anchoring element out the distal end of the cartridge. The plug may be formed from lyophilized hydrogel and the anchoring element may be formed from air-dried hydrogel, the anchoring element hydrating slower than the plug when exposed to an aqueous environment. During use, the plug and anchoring element are delivered into the puncture, the plug is cinched against the vessel wall. Protrusions on the anchoring element engage tissue surrounding the puncture to prevent proximal movement of the plug.
