A surgical apparatus for tightening a strap loop to a predetermined tension about split tissue portions includes a frame, an advancing mechanism for tightening the strap loop, a linkage mechanism which severs excess strap material not used in securing the tissue portions together and a control mechanism for controlling operation of the apparatus. The advancing mechanism includes a tensioning lever and a gear mechanism. The gear mechanism includes a tensioning hub and a clutch which releases when the tension in the strap loop exceeds a predetermined value. The control mechanism includes a safety feature for preventing inadvertent activation of the linkage mechanism. A buckle and a method for use with the surgical apparatus is also disclosed. The invention is particularly contemplated for repair of the human sternum when the sternum has been split during surgical procedures.

An apparatus for applying a buckle assembly about split portions of tissue is provided. The apparatus is particularly contemplated for use with a buckle of the type having a base member with an opening extending generally transversely therethrough for reception of a first strap end portion and a clamp member slidably housed within a channel formed in the base member. The apparatus includes a frame member, a mechanism disposed in a lower surface of the frame member for releasably supporting the buckle member and a mechanism associated with the frame member for advancing the clamp member within the channel of the buckle to a strap securing position wherein the strap member is secured within the buckle member in a looped configuration about the tissue portions.In an alternate embodiment, a surgical system for repair of split portions of tissue includes a flexible strap member having a buckle member attached thereto. The buckle member includes a base member having a transverse opening extending therethrough for reception of a first end portion of the strap member, a clamp member slidably housed within a partial longitudinal channel defined between lower and upper surfaces of the base member and a tab member for retaining the clamp in a secured position. The buckle may also include a clip member pivotally mounted to an upper surface of the base member to assist in securing the strap member. The system also includes an applier mechanism having a frame, a mechanism disposed in a lower surface of the frame for releasably mounting the buckle and a mechanism for advancing the clamp member within the partial longitudinal channel of the buckle to an engaged position whereby the clamp member securely wedges the strap against the base member.

The strap assembly includes a flexible strap member and a buckle member to secure the strap member. The buckle member includes a base member having an opening extending therethrough for reception of the strap member and a clip member pivotally mounted to the base member for engaging the strap member. The clip member is adapted to pivot from an open position to allow for reception of the strap member through the opening and a closed position wherein the strap is engaged by and wedged between a forward edge of the clip member and the base member to secure the strap member around the split portions of tissue.

An endoscopic surgical stapling apparatus is disclosed. The apparatus comprises a frame and a generally elongated endoscopic section connected to the frame and extending distally therefrom. The endoscopic section includes a mechanism for storing a plurality of staples, a mechanism for advancing the staples for positioning adjacent body tissue and a mechanism for at least partially closing each staple. A low pressure pneumatic system disposed in the frame provides the forces to at least partially activate the staple advancing mechanism. The apparatus has particular application in attaching mesh to body tissue as in laparoscopic hernia repair.

A detachable anvil assembly for use with an apparatus for performing circular anastomosis of tubular organs includes an elongated anvil rod having proximal and distal end portions and an anvil head mounted to the distal end portion of the anvil rod. The distal end portion is pivotally mounted and is adapted to pivot approximately plus or minus ninety degrees relative to a longitudinal axis defined by the rod. An elongated delivery member detachably receives the anvil assembly and facilitates delivery of the anvil assembly to the operative site. The pivoting feature of the distal end reduces the transverse profile of the assembly which consequently facilitates introduction and advancement of the anvil assembly within the tubular organ.
