摘要: A method of creating an AV fistula between adjacent first and second vessels includes inserting a guidewire from the first vessel into the second vessel. Inserting a catheter including a proximal member and a distal member over the guidewire such that a distal tip of the distal member comes into contact with a selected anastomosis site. Moving the distal member and the proximal member together to clamp tissue surrounding the aperture between the distal face of the proximal member and a proximal face on the distal member. Applying energy to an active heating member on the proximal member to cut and form the aperture, and to weld the edges thereof in order to create a desired fistula between the two vessels. Using a passive heating member disposed on the distal member to create a heating gradient across a tissue contacting surface of the distal member.

摘要: Various embodiments of a device for deep surgical corridor microvascular anastomosis are described herein. A device, comprising: an elongated body; a rod disposed at least partially within the channel of the elongated body; an actuator in association with the elongated body and the rod, wherein the actuator is operable to actuate the rod in a first axial direction; and a pair of coupling arms in association with the distal portion of the rod, wherein the actuator is operable to actuate the rod in a first axial direction such that the pivotable end of each coupling arm of the pair of coupling arms is consequently actuated in the first axial direction such that the pair of coupling arms are parallel with one another in a closed configuration.

摘要: A catheter-based device tracks over a guidewire which has been placed from a first blood vessel into a second blood vessel. The distal tip of the catheter is advanced into the second vessel while a proximal member remains in the first vessel. Matching blunt tapered surfaces on each of the distal tip and the proximal member are clamped together, with adjacent walls of each vessel between them, after which a known, controlled pressure is applied between the two surfaces. Heat energy is then applied to the blunt surfaces for approximately 1-30 seconds to weld the walls of the two vessels together. After coaptation of the vessel walls, the heat is increased to then cut through the vessel walls to create a fistula of the desired size.

摘要: Methods, devices, apparatus, assemblies, and kits for performing a vascular anastomosis are disclosed. A device for a vascular anastomosis includes tissue engaging portions that can move between at least two configurations. In some embodiments, the tissue engaging portions move without the aid of moving parts, while in other embodiments the tissue engaging portions extend from one or more movable wings. The tissue engaging portions may be separated by a first distance when in a pre-deployment configuration and by a second distance when in a deployed configuration. A method includes engaging a plurality of tissue engaging members of a coupling device against first end tissue. After selectively engaging the tissue engaging members and first end tissue, the first end tissue is stretched by at least moving the tissue engaging members. The stretched first end tissue is coupled to the second end tissue by mating the coupling device to a mating anastomosis device.

The surgical instrument includes a handle assembly, an elongated portion extending distally from the handle assembly and defining a longitudinal axis, and an end effector disposed adjacent a distal portion of the elongated portion. The end effector includes a first jaw member and a second jaw member. The first jaw member defines a first axis, which is transverse to the longitudinal axis. The first jaw member is pivotable about the first axis. At least one of the first jaw member and the second jaw member is movable with respect to the other between a spaced-apart and an approximated position.

摘要: An electrosurgical instrument for making an end-to-end anastomosis between two hollow organ sections includes two tools movable relative to each other and each including an HF electrode by which the hollow organ sections can be fusion-welded to each other. The two tools are substantially sleeve-shaped or can at least be brought into sleeve shape so that a first tool can enclose a first hollow organ section and a second tool can enclose a second hollow organ section. Each of the electrodes is formed on an end face of each sleeve-shaped tool around which the respective hollow organ section can be everted inside out, and the two tools are movable relative to each other so that the electrodes are aligned and can clamp the everted hollow organ sections therebetween.

摘要: A vascular anastomosis device comprises two rings with pins and an openable ring holder, which is made of a pair of components and detachably holds the rings. The end portion of a large-diameter blood vessel is inserted through a ring and the vascular wall is extended outward and impaled on the pins, the end portion of a small-diameter blood vessel is inserted through the other ring, and an incision is made from the end surface of the small-diameter blood vessel. The two rings are elliptically deformed so that the minor axis agrees with the diameter of the small-diameter blood vessel, the small-diameter blood vessel is elliptically opened and the vascular wall is extended outward and impaled on the pins, and the two rings are joined together by allowing the pins provided on the ring in one component to stick into the ring in the other component.

摘要: A surgical instrument that includes an elongated shaft that defines defining a central axis. The elongated shaft may have a distal end portion that is configured to operably support a circular staple cartridge therein. A tissue acquisition shaft may be axially movable within the elongated shaft such that a distal end portion of the tissue acquisition shaft may be distally advanced beyond the distal end portion of the elongated shaft. At least one tissue acquisition member may be pivotally attached to the distal end portion of the tissue acquisition shaft such that at least one tissue acquisition member is selectively pivotable about a corresponding acquisition axis that is substantially parallel to the central axis from a retracted position to deployed positions upon application of a deployment motion thereto. Various embodiments include an annular cutting member that is supported by the distal end of the elongated shaft for selective axial travel relative thereto.

The jaw assembly includes first and second jaws having arcuate grasping portions for securely grasping a circular surgical stapler anvil shaft. The jaw assembly is mounted on a movable head portion which is movable relative to a longitudinal axis of the elongate tubular member to rotate and articulate the jaw assembly relative to the longitudinal axis to manipulate the anvil within the body of a patient.

摘要: The present invention provides docking heads to be mounted on a graft so as to establish a vascular device that is coupled to a blood vessel with aneurysm, and dedicated delivery devices as well as methods of coupling. The vascular device comprises a graft having docking head at the proximal portion and another docking head at its distal portion, or two docking heads if the graft is bifurcated. The docking heads comprises a hollow truncated cone having a passage adapted to correspond the outer diameter of the graft and is provided with a plurality of outwardly pointing and inclined barbs. The vascular device is coupled to the blood vessel on both sides of the aneurysm while the docking heads act as guiding, anchoring and sealing means in a suture-less and rapid manner. The vascular device is modular and can be prepared according to the condition of the aneurysm and the dimensions of the blood vessels during operation.
