An implantable lead fixation apparatus, including two suture collars connected by a flexible retaining member of predetermined length, is provided for anchoring an elongate cylindrical electrode lead to a patient's body. Each suture collar comprises a flexible annular member having a central lumen that is adapted to enclose the lead. The suture collars each include provision for securing the lead against longitudinal movement of the lead with respect to the collar, and provision for anchoring the collar to the patient's body. The suture collars are positioned relatively proximally and distally with respect to the lead, and are secured on the lead with the proximal suture collar being separated from the distal suture collar by slightly less than the length of the retaining member. Then, the lead fixation apparatus is positioned with respect to the patient's body and secured to the body so that the proximal suture collar is located at a selected distance less than the predetermined distance from, and at an angular orientation to, the distal suture collar to provide slack in both the lead and the retaining member in order to accommodate patient movement without applying dislocating forces to a distal portion of the lead.

A source of antitachycardia pacing therapy and an electrode system including at least three electrodes for delivering the antitachycardia pacing therapy to the heart are provided. Circuitry and software for detecting a tachycardia having a site of focus in the heart and for determining the relative distances of the electrodes from the focus site are also provided. The three or more electrodes are connected to the source of antitachycardia pacing therapy based on the relative distances determined and in such a manner as to create a virtual electrode at the focus site upon delivery of the therapy to the heart.

A cardioverting/defibrillating pacemaker with the ability to sense and respond to haemodynamic compromise. The ventricular pressure (VP) and electrical activity of the heart are sensed via one or more transvenous or epicardial devices. The filtered peak-to-peak amplitude (VFPPA) or a peak pressure function (VPPF) based upon the right and/or left VP waveform(s) are derived. In one embodiment the VP and the electrical activity of the heart are sensed and processed continuously, and the VFPPA or the VPPF continuously derived. The VFPPA or the VPPF, and the ECG are used to initiate bradycardia pacing, antitachycardia pacing and cardioversion/defibrillation therapies. In another embodiment the VP and electrical activity of the heart are continuously sensed. The electrical activity is processed continuously and used to determine whether or not some irregularity exists. If so, one of the VPPF or VFPPA is derived and used to decide which of the therapies are to be used. One of the VPPF or VFPPA is also periodically determined to ensure optimal haemodynamic function. In both emobidments the degree of haemodynamic compromise is determined by comparing the derived VFPPA or VPPF with programmed values. An algorithm maximizes the effectiveness of support pacing. Preferably an ECG transvenous sensor in the right atrium and a combined ECG/VP sensor in the right ventricle are used.
