Abstract:
Full-thickness excision of tissue of a lesion in a lumen is performed through a natural orifice. Specifically, the tissue surrounding the lesion that is in a folded state is joined and severed using a linear stapler. The linear stapler includes a cutter and is inserted into the lumen. Joining and cutting is performed while organs outside of the lumen are pushed in a direction away from the lesion with the tip of the linear stapler. As a result of these steps being repeatedly performed along the periphery of the lesion, the lesion is excised.

A thread-fixing tool includes a first member in which a lumen opened at an end surface through which a suture thread is inserted, and a communication path having a first end opened at the end surface and a second end in communication with an intermediate portion of the lumen are formed, and a second member having a shaft portion inserted into the communication path of the first member, and a pressing portion which is formed at a distal portion of the shaft portion and which is capable of deforming outwardly in a radial direction of the shaft portion, wherein the pressing portion is guided to the intermediate portion of the lumen through the communication path, and deformed in a direction crossing an insertion direction of the suture thread to push the suture thread against an inner wall of the lumen.

This suture thread pushing apparatus moves a connection portion formed by one end portion and the other end portion of a suture thread used for suturing tissue within a body, which are guided to an outside of the body via a natural orifice, to a sutured portion of the tissue, and includes a flexible outer cylinder, and a flexible shaft member inserted into the outer cylinder so as to advance and retreat, wherein a pair of side holes facing each other is formed in a distal side of the outer cylinder such that the suture thread passes through the side holes, an engaging portion extending in an axial direction of the outer cylinder is formed in an inner surface on the distal side, and an engaged portion engaging with the engaging portion and sliding along the engaging portion is formed on an outer surface of the shaft member.

A tissue resection apparatus includes a pair of grasping members that holds a tissue; and a cartridge that is replaceably attached to the pair of grasping members. The cartridge includes a cartridge body that has a longitudinal axis and a slot extending along the longitudinal axis, guides a cutting member cutting a tissue by the slot, and defines a movement range of the cutting member; a pair of sealing portions that are provided on both sides of the slot so as to sandwich the slot and extend parallel to the slot, respectively; and a tissue sealing portion that is located closer to a distal end side than the slot and is provided on an extension line of the slot.

This tissue fastening apparatus is a tissue fastener for clamping first biological tissue and second biological tissue so as to be in close contact with each other, including: a first tissue fixation portion, made of an elastic wire wound in a coil, that is locked on the first biological tissue; and a second tissue fixation portion, made of an elastic wire wound in a coil, that is locked on the second biological tissue, the second tissue fixation portion continuing into the first tissue fixation portion, in which when falling off first and second biological tissue necrotized by being clamped between the first tissue fixation portion and the second tissue fixation portion, the tissue fastener moves only from the second tissue fixation portion to the first tissue fixation portion side and falls off.
