An example multilayer form includes a first layer with a first front face and a first back face, a second layer with a second front face and a second back face, and a third layer with a third front face and a third back face. The example multilayer form also includes a first line of weakness through at least the second layer and the third layer, and the three layers are coupled together between a first end and the first line of weakness. In addition, a second line of weakness extends through at least the first layer and the third layer, and at least the first layer and third layer are coupled between the second line of weakness and a second end.

Abstract: A document capable of providing information under the application of heat, which comprises a first support having a first surface and a second surface, the first surface bearing an information area including a visible principal image, and at least one of said first support or second support bearing at least one localized coating comprising a substantially colorless, non-pressure sensitive, heat-activatable chromogenic composition capable of producing a first color under the application of heat, the chromogenic composition comprising a chromogenic compound and a color developer. The chromogenic compound and the color developer are substantially colorless solids in physical contact prior to reaction, but which can chemically react to produce a visible colored image by application of heat at temperatures above room temperature. The information area is substantially free of chromogenic compounds, and free of color developers capable of reacting with chromogenic compounds to produce a visible colored image by application of heat.

Abstract: An assembly and method for making a card intermediate in which a series of spaced apart two layer laminates are arranged with the layers in face-to-face relation with a rupturable adhesive therebetween and a first of the layers being a transparent film, advancing the laminates toward a uniting station, providing a length of imprintable material having first and second surfaces with spaced areas of release material on the first surface and advancing the length toward the uniting station, adhesively uniting in the station the first layer of the series of laminates to the length in said spaced areas with each laminate extending beyond its united areas, and diecutting the length and the first layer to provide a generally rectangular closed perimeter diecut, the diecut also having a segment terminating short of the first layer and dividing the perimeter into two equal portions with one of the portions being within the area.

Abstract: An integrated pressure sensitive adhesive backed label assembly includes a security device such as an electronic article surveillance device, security tag or other antitheft device on a bottom side of the label so that the security device is not readily noticeable to the consumer. The label assembly can have the security device bonded to the adhesive backing of the label by separating the release liner from the label, placing the security device on the label and reapplying the release liner. Alternatively, as portions of the release liner can be cut and removed to expose a small area of the adhesive so that the security device can be attached directly to the label. The small area of the release liner can be cut and the security device attached to the cut portion so that the remaining portion of the release liner can be separated while leaving the cut portion and the security device attached to the label.

Abstract: A label assembly includes a label having pre-printed indicia or a printable face on an information side and a pressure sensitive adhesive on a back side. The label is releasably adhered to a front side of a transparent sheet. The transparent sheet includes a pressure sensitive adhesive on a back side thereof releasably adhered to a carrier sheet. The label assembly includes a label that can be printed by the ultimate user. The label can then be removed from the transparent sheet and applied to an article. The transparent sheet can then be removed from the backing sheet and applied over the label to provide a protective cover for the label.
