    摘要: A linear surgical stapling instrument includes a magazine for holding a plurality of undeformed surgical staples, and an upper jaw carrying the magazine and having opposed distal and proximal ends. An anvil is formed with staple-deforming pockets, and a lower jaw carries the anvil and has opposing distal and proximal ends respectively configured to mate with the distal and proximal ends of the upper jaw thereby to place the magazine and the anvil in confronting relation. A staple driver is carried in the upper jaw for driving staples held therein toward the anvil. Further, the upper and lower jaws are configured such that, when their respective distal and proximal ends are mated, a gap exists between the jaws in an intermediate region between their respective distal and proximal ends. Finally, a pivotably mounted clamp bar joins the upper and lower jaws and when actuated compresses them together in the intermediate region thereby to narrow the gap and to increase the clamping pressure between the magazine and the anvil. The instrument also includes a blocking system that prevents the jaws from being remated, once having been mated and having had staples in the magazine driven toward the anvil. Still further, the instrument includes a coupling device for coupling the jaws and clamp bar together when the staple driver is actuated.

    摘要: A laparoscopic ligation clip applicator for automatically applying a plurality of ligation clips includes a handle assembly having an actuator, an elongated shaft connected to the handle assembly, and an actuating mechanism disposed within the shaft. The actuating mechanism includes a clamp for engaging a first piece of the ligation clip and a slidable driver for driving a second piece of the ligation clip relative to the first piece. The actuating mechanism also includes a hold for preventing rearward movement of the ligation clips, and an advancer for sequentially feeding the ligation clips. A slidable cam, operably connected to the handle assembly, actuates the clamp, the driver and the advancer.

A first aspect of the invention includes a vascular anastomosis staple. A first configuration has two parts: an anchor member, forming the attachment with the target vessel wall and a coupling member, forming the attachment with the bypass graft vessel. The anastomosis is completed by inserting the coupling member, with the graft vessel attached, into the anchor member. A second configuration combines the functions of the anchor member and the coupling member into a one-piece anastomosis staple. A second aspect of the invention includes an anastomotic fitting, having an inner flange over which the graft vessel is everted and an outer flange which contacts the exterior surface of the target vessel. A tailored amount of compression applied by the inner and outer flanges grips the target vessel wall and creates a leak-proof seal between the graft vessel and the target vessel. A third aspect of the invention has a flange to which the graft vessel attaches, by everting the graft vessel over the flange, and a plurality of staple-like members which attach the flange and the everted end of the graft vessel to the wall of the target vessel to form the anastomosis.
