The present invention provides a method and apparatus for detecting and treating sleep respiratory events that includes a plurality of sensors gathering physiological data related to sleep respiratory events. A processor extracts an average cycle length and a frequency of at least one of Cheyne-Stokes respiration and periodic breathing based upon the physiological data, and determines whether therapy is required based on the average cycle length and the frequency.

The invention relates generally to a system and method for monitoring and automatically delivering a therapy for sleep-related disordered breathing. In one form the present invention relates to an external device for monitoring for sleep-related disordered breathing in communication with an implantable medical device for delivering an electrical stimulation therapy. In another form the present invention relates to an implantable medical device for detecting sleep-related disordered breathing episode(s) and an external apparatus (e.g., a CPAP machine) for providing therapy to terminate, and/or reduce, said episode(s). In this form of the invention, the implantable medical device communicates with the external apparatus so that the therapy provided corresponds in magnitude and duration to the severity and/or length of the episode(s). In yet another form, an implantable apparatus detects said disordered breathing episode(s) and a hybrid therapy is provided by both the implantable apparatus and an external apparatus.

A method and apparatus for cardiac pacing are provided in which pacing pulses are delivered at an increased rate in response to a detected intrinsic heart rate drop and a special rate drop response detection scheme is temporarily disabled until an intrinsic heart rate exceeds a predetermined threshold value. If the pacing rate reaches the lower pacing rate without sensing intrinsic activity, heart rate drop detection remains disabled and lower rate pacing continues. Rate drop detection is re-enabled whenever sufficient sensed intrinsic activity indicates that a sudden intrinsic rate drop could occur again. Thus, the subsequent reduction in heart rate requiring therapy is declared only if a sensed intrinsic heart rate drops from above an intermediate value (herein defined as a re-enable rate) that is set above the lower pacing rate and an upper pacing rate.

A method of and apparatus for determining the physical posture of a patient's body, having a superior-inferior body axis, an anterior-posterior body axis and a lateral-medial body axis, in relation to earth's gravitational field. A medical device having first, second and, optionally, third accelerometers having sensitive axes mounted orthogonally within an implantable housing is adapted to be implanted with the sensitive axes nominally aligned with ideal X, Y and Z device axes correlated to patient body axes. Each accelerometer generates DC accelerometer signals having characteristic magnitudes and polarities on alignment of the sensitive axis with, against or normal to earth's gravitational field and DC accelerometer signals of varying magnitudes and polarities when not so aligned. The actual pitch and roll angles of the sensitive axes of the accelerometers in the implanted IMD with respect to the true gravitational axes are determined and the yaw angles are determined or estimated. Corrected DC accelerometer output signals are derived from the pitch, roll and yaw angles and are compared to a set of stored thresholds for each body posture to determine the actual body posture. Storage of these comparisons or the raw signals themselves can be used for histogram data to provide theraputic or research benefits also described.
