New chemotherapeutic medicaments and certain medical uses and methods for use of such chemotherapeutic medicaments for treatment of disease in human or animal tissue are described, wherein a primary active component of such medicaments is a halogenated xanthene or halogenated xanthene derivative. Preferably, the halogenated xanthene is Rose Bengal or a functional derivative of Rose Bengal. The halogenated xanthenes constitute a family of useful chemotherapeutic agents that afford selective, persistent accumulation in certain tissues. In preferred embodiments, such medicaments are used for treatment of a variety of conditions affecting the skin and related organs, the mouth and digestive tract and related organs, the urinary and reproductive tracts and related organs, the respiratory tract and related organs, the circulatory system and related organs, the head and neck, the endocrine and lymphoreticular systems and related organs, various other tissues, such as connective tissues and various tissue surfaces exposed during surgery, as well as various tissues exhibiting microbial or parasitic infection. Apparatus formed of a laser lucent hollow, transparent needle (10) with an optical fiber (12) disposed within its interior. Means such as a spacer (14) can be used to retain the fiber in position within the needle. In a particular embodiment, the end of the needle adjacent to the energy transmitting end of the fiber is closed, and energy scatterers or diffusers (18) may be provided to alter the characteristics of the transmitted energy. In further embodiments, scattering or diffusing materials (32) are disposed within the walls and the closed end (16) of the needle to form a laser lucent apparatus customized to specific hyperthermia requirements. In other embodiments, the needle is open-ended for insertion of a specialized instrument for such uses as biopsies and spectroscopic tissue analyses. These procedures can be conducted before or after hyperthermia treatment of the tissue by replacing fiber in the needle casing with such a specialized instrument or vice-versa. A particular application facilitates the use of laser-induced hyperthermia to treat benign prostatic hypertrophy.

A method for the treatment of atherosclerosis in a mammal by destruction of atheromatous plague. The disclosed method includes injecting a hematoporphyrin into the mammal for selective uptake into the atheromatous plague (16), and delivering light to the diseased vessel (10) so that the light activates the hematorporphyrin for lyses of the plague (16). The preferred method utilizes a balloon catheter (14, 12) equipped with flexible optical fibers (20) for transmission of light from an external source for illumination of the interior of the inflated balloon (14). By inflation of the balloon (14), the opaque blood between the balloon (14) and the atheromatous plague (16) is displaced to facilitate activation of the hematoporphyrin. The balloon (14) may be illuminated and inflated and deflated in a cycle responsive to the patient's pulse so as to minimize interference with blood flow.

Percutaneous methods and systems for treating an intervertebral disc are disclosed. An intervertebral disc treatment apparatus includes an introducer (12) and a catheter (14) having a distal end (28), a proximal end, a longitudinal axis, and an intradiscal section (16) at the distal end on which there is at least one functional element (29). The treatment proceeds by applying a force longitudinally to the proximal end of the catheter which is sufficient to advance the intradiscal section through the nucleus pulposus and around an inner wall of an annulus fibrosus, but which force is insufficient to puncture the annulus fibrosus. The functional element is positioned at a selected location of the intervertebral disc by advancing or retracting the catheter and optionally twisting the proximal end of the catheter. Then, the functional unit treats the intervertebral disc. Optionally, there is an additional step of aspirating and/or adding a substance to the intervertebral disc.
