    摘要: A catheter adapted to increase the patency of a body conduit includes an elongate tube having an axis extending between a proximal end and a distal end, and a balloon disposed at the distal end of the tube and having properties for being expanded to a high-profile state and for being contracted to a low-profile state. A sleeve disposed over the balloon has a pair of ends disposed on opposing sides of a central section, the ends having a floating relationship relative to the tube with the central section disposed circumferentially of the balloon. An electrode disposed outwardly of the sleeve has properties for being electrosurgically energized to incise materials defining the body conduit when the balloon is in the high-profile state. The electrode can be formed of a plurality of elements stranded to increase the surface area of the electrode. The catheter can be inserted relative to a guide member having a conductor which carries the electrosurgical energy from the proximal end of the tube to the electrode at the distal end of the tube. An associated method includes the step of introducing electrosurgical energy into the conductor of the guide member to energize the electrode of the catheter.

A valve housing attached to the proximal end of the cannula forms a working channel with the cannula and includes a rigid housing portion and a flexible housing portion. First and second septum valves are formed in an end wall of the flexible housing portion and form separate valve sets with associated zero closure valves disposed within the valve housing. The valve sets are sized and configured accommodating different ranges of instrument diameters. The first and second septum valves are offset from the axis of the cannula in normal positions but are movable to positions more aligned with the axis of the cannula, due to elastomeric characteristics of the flexible housing.

    摘要: A suturing apparatus adapted to close an incision and an anterior tissue wall includes an elongate support structure having an axis extending between a proximal end and distal end. A receiver disposed at the distal end is insertable through the incision in a low-profile state and expanded to a high-profile state on the far side of the tissue wall. Needles carried by the support structure are operable to extend through the support wall on either side of the incision and into the expanded receiver. Sutures advanced through the needles are captured by the receiver after the needles are removed and the receiver is returned to its low-profile state. Removal of the suturing apparatus from the incision draws the captured suture ends outwardly through the incision. A novel suture knot can be tied in the four suture ends and moved into proximity with the tissue wall to close the incision.

    摘要: A surgical device is disclosed for treating and removing obstructing material from within a vascular conduit or other body passageway. The device comprises a catheter device having an expandable and collapsible outer mesh sleeve disposed at a distal end. The outer mesh sleeve is configured such that when expanded in the obstructed conduit or passageway, a portion of the obstructing material is forced through a plurality of openings in its outer surface and thus, into the outer mesh sleeve. An inflatable and deflatable balloon may be coupled to the catheter and used to radially expand the outer mesh sleeve into the obstructing material. A treatment element disposed within the catheter device is moved against the inner surface of the outer mesh sleeve to treat and remove any obstructing material with which it contacts. The treatment element may be configured as a blade for physically removing the obstructing material, or as an electrode for electrosurgical treatment. After treatment or removal of the obstructing material, the outer mesh sleeve is collapsed and the catheter device removed from the body conduit.

    摘要: An obturator adapted for use in penetrating a body wall of a patient includes an elongate shaft having an axis extending between a proximal end and a distal end. A handle is disposed at the proximal end and an arcuate cutting edge is formed at the distal end of the shaft. The shaft may include a plurality of inner walls and passages, with a shield mechanism movable within the passages from a retracted cutting position to an advanced position. A system of flanges and walls can be used to maintain the shield elements in close proximity to and sliding engagement with the associated inner walls. In an associated method of manufacture, the shaft of the obturator can be extruded.

    摘要: The invention is directed to a surgical access system that provides access to a surgical area while maintaining pneumoperitoneum during laparoscopic surgery. The access system comprises a sheath retractor adapted to dilate a wound stretchable to a desired diameter and a detachable seal adapted to be removable from the sheath retractor. In another aspect, the detachable seal comprising a valve including a plurality of overlapping sheets defining an access channel that extends into communication with the incision in the patient. Each of the overlapping sheets includes a portion of the perimeter that is not fixed to the inner diameter of the ring, which provide open edges defining the access channel.

    摘要: The disclosed stent is formed of an elongate, flexible duct having a very thin wall and a preformed diameter, length, and shape. The stent is constructed of a woven tubular structure of multiple strands or elements. The woven tubular structure is thermally set to a predetermined diameter and length, so that the “at rest” or natural condition of the tubular structure is predictable. A retention or holding member can be formed at one or both of the ends of the stent. This retention member can be reduced in diameter for insertion into the body passage. The woven tubular structure provides a path for fluids to flow in and around the stent, while a patent lumen is being developed. The woven tubular structure allows the stent to be extended or stretched over a guidewire or other noncompressive member, to thereby reduce the diameter of the stent for insertion of the stent into a body passage.
