摘要: A surgical implant for adhering two portions of tissue together with a polymer adhesive is disclosed. The surgical implant has a matrix structure with one or more layers and a plurality of holes for tissue growth therethrough. The matrix structure controls placement of the adhesive to minimize adhesive area and maximize tissue regrowth areas. In addition, the surgical implant can include drugs and adhesive initiators, and can include multiple layers of structure with any combination of holes, drugs, adhesives and adhesive initiators within. Additionally, the surgical implant can be surrounded by a rapidly dissolving pouch to prevent unwanted polymerization of the adhesive prior to placement in the body.

摘要: An apparatus for regulating the functioning of a patient's organ or duct includes an elongated member having a first end and a second end. A fastener is disposed on the first end of the elongated member. The fastener is configured to engage the second end of the elongated member so that the elongated member forms a loop around the organ or duct. A tension element is disposed for movement within the elongated member. A symmetrical drive system including a drive element associated with and engaging the tension element for causing the tension element to control the tension applied by the elongated member against a patient's body organ or duct is further provided. In accordance with an alternate embodiment an apparatus for regulating the functioning of a patient's organ or duct including an elongated member having a first end and a second end. A fastener is disposed on the first end of the elongated member. The fastener is configured to engage the second end of the elongated member so that the elongated member forms a loop around the organ or duct. A tension element is disposed for movement within the elongated member. The tension element is composed of a flexible first member and a flexible second member, wherein the flexible first member is movable relative to the flexible second member. A drive element is associated with and engages the tension element for causing the tension element to control the tension applied by the elongated member against a patient's body organ or duct.

摘要: A circular stapling instrument is provided which includes a reciprocating drive shaft and a firing bar. The reciprocating drive shaft has a first engagement member at a tip of the reciprocating drive shaft and a threaded adjustment member at a base of the reciprocating drive shaft. The firing bar is movably connected at a first end with a firing trigger and movably connected at a second end with a stapling head assembly through a second engagement member. The firing bar forms a first engagement surface which faces a respective second engagement surface formed on the reciprocating drive shaft. The circular stapling instrument also includes a frictional reducing member located in between the first and second engagement surfaces in order to reduce friction between the reciprocating drive shaft and the firing bar.

摘要: A stapling reload assembly for a circular stapling instrument includes a casing, a staple holder, a staple driver, and an anvil assembly. The staple holder is attached to the casing. The staple driver includes a driver alignment surface. The staple driver is receivable within the casing and is configured to move within the casing from a pre-fired position to a fired position. The anvil assembly includes an anvil base surface, an anvil alignment surface, and a staple forming surface. The anvil base surface is configured to move from an open position away from the casing to a closed position adjacent the casing. The anvil alignment surface is configured to mate with the driver alignment surface, to rotationally align the staple forming surface with the staple holder, when the anvil base surface is in the closed position.

摘要: A fastening device for clamping and fastening tissue together with fasteners formed from an adhesive is disclosed. An embodiment of the adhesive fastening device can clamp a first and a second portion of tissue together between a first and a second jaw, and can place a fluid polymer adhesive into tissue and form it into a polymerized surgical fastener within the tissue. The adhesive fastener can be formed as a polymerized shank or shaft extending through the tissue, and can have one or more heads formed on the shank. The shank can be formed solely from the polymerizable adhesive, or can be formed from another material such as nitinol, titanium, stainless steel, suture, or a plastic. Shanks of other material can be combined with the adhesive. Alternately, an adhesive fastener can be formed extending distally from a distal end of the surgical device. And, an adhesive fastener can be formed around a luminal tissue or within luminal tissue.

摘要: An articulating surgical instrument which has an articulation transmission assembly for remotely articulating an end effector of the instrument is disclosed. Ratcheting rotation of a compressible deck within a detent housing of the assembly from a first locked position to a second locked position causes axial movement of an elongated transmission member for the articulation of the end effector from a first position to a second position angled from the first position. Also disclosed is a flexible neck assembly for articulating the end effector of the surgical instrument. Neck ribs, each having an interior plate and an exterior dish, extend from a central longitudinal rib separating a pair of flexible neck portions of the assembly. A pair of transmission band assemblies, each having an interior articulation band and an attached exterior reinforcement band, reciprocate in opposite directions within side slots of the neck ribs in response to actuation of the articulation transmission assembly. Reciprocation causes the flexible neck assembly to bend, effecting articulation. The shape of the neck ribs optimizes the radius of curvature when articulation is effected, and the reinforcement bands prevent buckling of the articulation bands.
