摘要: A method and apparatus for cardioverting the atrium of a human heart that includes insertion of first and second elongated electrodes tranvenously into the heart and associated vessels. One electrode is preferably located in the coronary sinus and great vein of the heart. The other electrode is preferably located in the vicinity of the right atrium of the heart, spaced from the electrode located in the coronary sinus. In response to detection of fibrillation or in response to manual triggering, a defibrillation pulse is applied between the first and second electrodes to effect atrial cardioversion. Further, after delivery of a successful defibrillation shock, the width of intrinsic p-waves are monitored and bi-atrial pacing is temporarily initiated if the width exceeds a preset or programmable threshold.

摘要: A non-tissue contacting electrode system senses physiologic signals from a patient during implant and/or followup of an implantable medical device (IMD) via an external programmer or other monitoring instrument. These sensing systems are electrically connected to the circuitry of the external device and detect cardiac depolarization waveforms displayable as electrocardiographic tracings on the instrument screen when the programming head is positioned above an implanted pacemaker (or other implanted device) so equipped with a non-tissue contacting electrode system. The structure and system provide an enhanced capability for detecting and gathering physiological signals from a patient with minimally invasive patient contact.

摘要: Epidermally mountable device includes terminus structures to enable positive connection with conductive tissue. The device includes nano spikes shaped to penetrate the epidermis of the skin to collect electrical biopotentials such as cardiac depolarization waveforms (ECGs) and various signals transmitted by implanted devices. Generally, the nano spikes are integrated on a substrate comprising, preferably, a flexible metalized conductive plate attachable to an adhesive backing. The device could be mounted on any external body of the patient to collect ECG, EEG or other signals. Further, the device is adaptable to include a programmable microprocessor, including memory and an antenna to store, receive and transmit data as needed. In one embodiment, in addition to collecting physiologic data through direct contact, the device may also be used as a smart patch to download, store and transfer data from implanted medical devices or other data sources using wireless data transmission medium.

摘要: A method and a system for retrieving information from an IMD so that a physician may better use the time allotted to a patient. In an example embodiment, a method for communicating between an implanted device and a medical data processing system occurs via a communications module coupled to an antenna member and to the medical data processing system. The communication module and the antenna are arranged to transmit and receive radio frequency signals within a given range or space such as in a room. The method includes broadcasting interrogation requests in the range via the communications module and antenna arrangement and establishing a communications link between the implanted device present in the range and the communications module. A set of patient diagnostic data is then transmitted from the implanted device to the communications module in response to an encoded radio frequency signal from the communications module. The set of diagnostic data is then processed and reported via the medical data processing system for use during a patient examination.

摘要: Methods and apparatus for communication of implantable medical device (IMD) information, including interrogation of programmed parameter values, operating modes and conditions of operation, confirmation of programmed changes thereof, interrogation of data stored in the IMD, and patient warnings or other messages by RF transmission of audible sounds generated by the IMD are disclosed. The IMD includes an RF transmitter that broadcasts or transmits audible sounds including voiced statements or musical tones stored in analog memory correlated to a programming or interrogation operating algorithm or to a warning trigger event. The broadcast radio signal is received, and the audible sounds are demodulated and reproduced by a radio receiver as voiced statements or musical is tones that convey human understandable messages comprising IMD information generated during programming and interrogation sessions and warnings or status messages to the patient at other times.

摘要: Power consumption in medical devices is reduced through the operation of circuits at clock speeds of lower levels to adequately complete desired functions during predetermined time periods (e.g., blanking interval, upper rate interval, etc.) just-in-time prior to subsequent required functional processes; by providing supply voltages tailored for various circuits of an integrated circuit; by operating two or more circuits of an integrated circuit at different clock frequencies; by changing the supply voltage level “on the fly” as required by specific circuit timing functions required for various circuitry based on clock frequencies used to control operation of such circuitry; and/or by tailoring back gate bias or adjusting back gate bias “on the fly” for circuits based on the supply voltage level applied to the circuits.

摘要: Methods and apparatus for communication of implantable medical device (IMD) information, including confirmation of programming and programmed parameter values, operating modes and programmed changes thereof and data stored in the IMD, by emission of audible sounds by the IMD are disclosed. The IMD includes an audio transducer that emits audible sounds including voiced statements or musical tones stored in analog memory correlated to a programming or interrogation operating algorithm or to a warning trigger event. The audible sounds can comprise the sole uplink transmission or may augment the contemporaneous uplink RF transmission of stored data, and/or programmed operating modes and parameters and/or device operations and states in an interrogation or during programming. To conserve energy, the audible sounds accompanying interrogation and programming of the IMD are at a low volume that preferably cannot be heard without use of an external audio amplifier or stethoscope. Voiced statements warning of battery energy depletion or imminent delivery of a therapy are emitted at a higher volume that is sufficient to be heard by the patient, so that the patient can take appropriate action. The volume of audible sounds confirming programmed changes made by a patient using a limited function programmer or magnet is also increased so that the patient can hear them.

摘要: An implantable cardioverter for providing cardioversion electrical energy to at least one chamber of a patient's heart in need of cardioversion and applying a pain alleviating therapy at an appropriate site in the patient's body prior to or in conjunction with the delivery of the cardioversion energy to the heart chamber to alleviate propagated pain perceived by the patient. The combined cardioversion and pain alleviating therapies are preferably realized in a single implantable, multiprogrammable medical device or separate implantable cardioversion and pain control devices with means for communicating operating and status commands between the devices through the patient's body.
