Abstract: An implantable medical device system is configured to generate signals representing activity of a heart of a patient; determine, based on the signals, an intrinsic delay of the heart of the patient; determine whether the intrinsic delay is indicative of a first-degree heart block being present in the heart of the patient; determine a patient-specific timing regime for conduction system pacing based on whether the intrinsic delay is indicative of the first-degree heart block being present in the heart of the patient; and administer cardiac pacing to a native conduction system of the heart of the patient based on the timing regime.

Abstract: A system for detecting an atrial tachyarrhythmia episode includes a medical device having sensing circuitry configured to receive a cardiac electrical signal from electrodes coupled to the medical device and a processor configured to detect an atrial tachyarrhythmia episode in response to a time duration of the cardiac electrical signal classified as an atrial tachyarrhythmia being greater than or equal to a first detection threshold. The processor is configured to determine if detection threshold adjustment criteria are met based on at least the detected first atrial tachyarrhythmia episode and adjust the first detection threshold to a second detection threshold different than the first detection threshold in response to the detection threshold adjustment criteria being met.

Abstract: A medical device system, such as an extra-cardiovascular implantable cardioverter defibrillator ICD, senses R-waves from a first cardiac electrical signal by a first sensing channel and stores a time segment of a second cardiac electrical signal in response to each sensed R-wave. The medical device system determines a morphology parameter correlated to signal noise from time segments of the second cardiac electrical signal, detects a noisy signal segment based on the signal morphology parameter; and withholds detection of a tachyarrhythmia episode in response to detecting a threshold number of noisy signal segments.

Abstract: An implantable cardioverter defibrillator (ICD) performs a method that includes determining whether first criteria for detecting a ventricular tachyarrhythmia are met by a cardiac electrical signal. The ICD determines features from cardiac signal segment of a group of cardiac signal segments and determines whether a first portion of the features satisfy monomorphic waveform criteria and determines whether a second portion of the features satisfy supraventricular beat criteria. The ICD determines whether second criteria for detecting the ventricular tachyarrhythmia are met and withholds detecting of the ventricular tachyarrhythmia in response to the monomorphic waveform criteria and the supraventricular beat criteria being met.

Abstract: A medical device is configured to deliver a shock to a patient's heart via electrodes coupled to the medical device and set an escape interval timer to start running an escape interval after delivering the electrical shock. A sensing module of the medical device is configured to sense a cardiac event in response to a cardiac electrical signal received by the medical device crossing a sensing threshold. The medical device determines if the cardiac event meets reset criteria and allows the escape interval timer to continue running the escape interval if the cardiac event does not meet the reset criteria.
