    摘要: A thoracic arch stent graft (1) has a tubular body of a biocompatible graft material (3) and at least one fenestration (13, 15) in the wall on one side thereof. A combination guide and tubular side arm assembly (20, 22) is mounted into the or each fenestration. The combination guide and tubular side arm assembly comprises a tubular portion (24) and a funnel portion (26) extending from a distal end of the tubular portion. The funnel portion has an angled open end. The tubular portion and the funnel portion are formed from a biocompatible graft material wall and have a lightweight space frame (38 40) formed from a resilient wire and supporting the graft material and defining the combination guide and tubular side arm assembly.

    摘要: A stent graft introducer has a pusher, a nose cone, a region between the pusher and the nose cone for a stent graft, a sleeve extending over the pusher and proximally to the nose cone to cover the stent graft when retained in the region. An indwelling auxiliary catheter with a pre-curved proximal end is fitted into the introducer. The nose cone has an elongate groove and the auxiliary catheter has its proximal end in the elongate groove. In a partially retracted position of the sleeve the curved proximal end of the indwelling auxiliary catheter is exposed and uncovered by the sleeve and in an advanced position of the sleeve the curved proximal end of the indwelling auxiliary catheter is straightened, extends along the length of the groove in the nose cone and is covered by the sleeve.

Retraction of the release wire closes the valve. The stent graft can have a tubular body with a first bifurcation (134) with first and second legs (130, 132) extending from the bifurcation. The side aperture and valve arrangement can be in one of the legs. One of the legs (132) has a further bifurcation (142) to define a side arm (140). The stent graft can be deployed into the vasculature of a patient with the tubular body being in an aorta of the patient, a first leg extending down an iliac artery, a second leg being directed towards a contralateral iliac artery and the side arm directed to an internal artery of the contralateral iliac artery.

    摘要: An introduction arrangement for a fenestrated or branched stent graft (13) intended for deployment into the lumen of a vessel having a blind vessel extending from it. The introducer (1) has a distal end intended to remain outside a patient in use and a proximal end with a nose cone dilator (11) and an arrangement to retain the branched stent graft distally of the nose cone dilator. A sheath (15) on the introducer extends over the branched stent graft to the nose cone dilator. An indwelling catheter (21) extends from the distal end of the introducer and enters the fenestration or side arm and through to the nose cone dilator, the indwelling catheter has a guide wire (29) extending through it. The guide wire can be extended beyond the nose cone dilator in use before the sheath is withdrawn from the branched stent graft so that it can be snared from the contra-lateral artery.

The prosthesis assembly has at least first and second members (2,4) with an end portion (12) of one member (2) to be joined to an end portion (24) of the other member (4) when in and when expanded within a lumen of a patient. Each member (2,4) has a stent arrangement (10,25) associated with a graft arrangement (8,26). The end portion of one member has at least part of its stent arrangement on the inner surface of its graft arrangement and the end portion of the other member has at least part of its stent arrangement on the inner surface of its graft arrangement. Advantageously two stents are overlapped. The joining portions of the first and second members can have the same diameter.

    摘要: A prosthesis (18) comprising stents (7,8) sutured to a graft (5) comprising a tube of biocompatible material. The stents are attached to the inside surface of the tube and there is at least one fenestration (10) in the tube corresponding to an intersecting artery opening. A further stent (1) extends proximally and may have caudally facing barbs (2). Each fenestration includes one or more radiopaque markers (11) defining a periphery of the fenestration. A release mechanism for the prosthesis includes trigger wires (15,17) stitched into a fold (16) on the material to retain the prosthesis in a partially compressed state during deployment. A delivery device (20) has a sheath (26) to hold the compressed prosthesis during insertion and a top cap (24) to retain the top stent (1).

    摘要: A reinforcing ring (10) for a fenestration (32) of a stent graft which can be surface treated such as by passivation and/or electropolishing. The reinforcing ring has several turns of a substantially inextensible resilient wire (12) in a circular two dimensional planar shape and terminal ends (14) at each end of the wire. The terminal ends each comprising a loop (14) and a tail (16). The tail is folded back and extends around the circular shape. Each of the tails of the terminal loops can have an enlarged end. The reinforcing ring can be straightened out for surface treatment such as passivation and/or electropolishing with substantially no part of the circular shape, the loops or tails touching each other.

The side arm (5) is stitched into an aperture (11) in the main tube and is in fluid communication with it. The aperture is triangular, elliptical or rectangular and the side arm is cut off at an angle to leave an end portion having a circumferential length equal to the circumference of the aperture. The side arm can also include a connection socket (76) comprising a first resilient ring (79) around the arm at its end, a second resilient ring (80) spaced apart along the arm from the first ring and a zig zag resilient stent (82) between the first and second rings. The zig-zag resilient stent can be a compression stent. Both the main tube and the side arm are formed from seamless tubular biocompatible graft material.
